Summary:
Under the supervision of the Chief Clinical Officer, and the Clinical Supervisor, with dotted line reporting to the Director of Child and Family Services, the Child Therapist specializes in early childhood development and provides counseling and therapy services to children under 8 years old and their families. Duties and Responsibilities of the Child Therapist (Included but not limited to) You'll report to the Clinical Supervisor and:

What You'll Do:
Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient's plan of care. Provides an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
Provides accurate and timely documentation of patient services to reflect the plan of care
Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
Serves as liaison between patients and families/caregivers and community agencies
Qualifications:
Social Work Degree
New Mexico: Master's Degree in Social Work required.
Current State Licensure in Social Work
New Mexico: LMSW or LCSW
Minimum of one year's experience in health care or hospice setting
Valid driver's license and proof of insurance is required

LCDN's mission is to provide culturally competent care to the underserved and medically indigent patients in a remote part of northern New Mexico. LCDN operates three clinic sites in Abiquiu, Ojo Caliente, and El Rito, all strategically located to provide maximum access to the area's rural residents and School-Based Health Centers in Pojoaque, Los Alamos, NM.
Daily Responsibilities
Intake, diagnose, evaluate, develop treatment plan, and monitor progress for patients to include individuals, couples, families, and groups
Establish working relationships with sponsoring, advisory, and related services agencies
Promote organization by ensuring the understanding of program services available to clients
Enforce rules, regulations, and legal requirements with clients and document events of the therapeutic process
Assist clients in obtaining services by initiating referrals to community agencies and resources
Work hours: Four (10) hour workdays
