RESPONSIBILITIES

Identify and recommend additional referrals as needed.

QUALIFICATIONS
Master's Degree in relevant human services field required.
LMSW, LCSW, LMHC or Limited Permit required.
At minimum of 1 year of related experience in a behavioral health setting required; clinic experience preferred.
Prior experience diagnosing using the DSM V and experience with behavioral/therapeutic interventions for children, adolescents, and adults preferred.
Proficient computer skills, including Microsoft Office and Electronic Medical Records, are required.
Excellent verbal and written communication skills are required.
Ability to work both independently, as well as on a team is required.
Strong judgement and problem solving skills and understanding of boundaries are required.
Flexibility to work evenings and/or Saturdays required.
Bilingual in Spanish preferred.

Treats and manages mental health and/or substance abuse problems. Assists with screening, identification, management, and treatment of victims of abuse, neglect, domestic violence, rape, etc. Provides training and consultative services to other health care professionals.
Arranges, procures, and coordinates patient and/or family pre and post hospital or treatment needs.
Provides education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment, and/or life situation. May utilize crisis intervention, brief and long-term individual, group, and family therapies. Provides teaching and training for students and staff from social work and other disciplines.
Participates in multi-disciplinary health care teams and provides leadership in representing the social work perspective to and being an advocate for patients and families.
Maintains records and statistics according to departmental and institutional guidelines and standards.
Attends and participates in staff, committee, departmental, institutional, and other administrative meetings and continuous quality improvement efforts.
Minimum Qualifications:
To qualify you must have a Master's Degree Social Work. Current and valid New York State Licensed Master SocialWorker (LMSW) license or Licensed Clinical SocialWorker (LCSW) license. 1 - 2 years Social work or social work internship in a healthcare setting or social service agency. Solid working knowledge of social work practice, relevant medical and/or legal issues that impact patient care, and medical center resources to access and provide for patient care needs. Excellent communication (both oral and written), interpersonal, assessment, counseling, multitasking, detail orientation, and organizational skills. The incumbent works closely with psychiatrists, psychologists and other health professionals in treatment planning, but is expected to work with limited supervision, exercising a considerable amount of judgment and initiative, especially in determining the appropriateness of the psychiatric services for applicants at intake. Supervision is received from a professional supervisor through review of case records and reports, and individual and staff conferences. Employees in this class who provide clinical services must be supervised by a Licensed Clinical SocialWorker (LCSW).

KEY RESPONSIBILITIES
:
Perform daily in person mental status evaluations to identified incarcerated individuals.
Provide services of long and short term supportive counseling to incarcerated individuals.
Provide Crisis Intervention.
Evaluate incarcerated individuals for possible hospitalization.
Refer incarcerated individuals for case management.
Provide Treatment planning, individual and group counseling.
Maintain accurate and timely documentation of progress.
Provide clinical /technical supports to staff members in the unit.
Refer incarcerated individuals for possible medication management.
Refer incarcerated individuals to medical or dental, if needed.
Perform Suicide Watch activities
Monitor incarcerated individuals on suicide watch.
As part of a multi-disciplinary team, which may include Psychiatrists, Psychiatric Nurse Practitioners, Psychologists and other treatment professionals; participates in case conferences and staff meetings to discuss, diagnose and appropriately treat patients;
Perform all other duties as may be assigned by the Supervisor.

NECESSARY SPECIAL REQUIREMENTS:
