**Job Description:**
+ Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
+ Assesses patients' and families' wholistically for discharge planning needs in the inpatient, observation and/or emergency departments, including prior functioning, support systems, financial, and psychosocial in a timely fashion to avoid delays in discharge planning.
+ Reviews the medical record, including medications, history and physical, labs, and progress notes and incorporates the clinical, social, and financial factors into the transition of care plan.
+ Develops discharge plans with appropriate contingency plans throughout the hospital stay to ensure timely care coordination and progression of care, making arrangements for post-acute care services and facilities as well as community care for social needs.
+ Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities to ensure patient care information is communicated for continuity of care, medical records are complete, and discharge reconciliation is accurate.

Patient Services Workers are responsible for a variety of specialized duties related to the receipt, interpretation, and follow-through of patient diet orders in hospital and long-term care settings. Acts as a liaison between the patient, the Food and Nutrition Services Department, and Nursing Services. This role requires working on Saturdays and Sundays.

Job Responsibilities
* Delivers and collects patient trays during meal service. Delivers nourishments and/or snacks to patients as ordered by physician/dietitian in a timely manner. Visits patients to introduce the services of the Food and Nutrition Services Department.
* May obtain food preferences/dislikes from patients and/or family members.
* Communicate all patient food needs to the appropriate area of the Food and Nutrition Services Department.
* Ensures that nutrition diet care orders are provided to the patient by offering appropriate menu selections.
* Maintains and adheres to all sanitation standards by following assigned cleaning schedules. Completes other sanitation tasks as assigned by the Supervisor/Relief Supervisor.
* Inventories and re-stocks pantries, refrigerator, and freezers on assigned unit(s).
* Maintains temperature logs for unit refrigerators and freezers.
* Understands therapeutic diets using established protocols and seeks assistance from Supervisor or Dietitian if an error is observed.
* Assists the clinical staff in ensuring all patients' basic nutrition care needs are met.

Job Summary:
Northwestern University Mesulam Center for Cognitive Neurology and Alzheimer's Disease (Mesulam Center) is seeking an exceptional socialworker who demonstrates a pronounced interest in working with persons with neurocognitive disorders and their families in a diversity of roles. The SocialWorker will work as a team member to provide clinical care for persons living with dementia and their families and achieve the goals and objectives of the Outreach, Recruitment and Community Engagement (ORE) Core, the Miller Family Buddy Program, in addition to other psychosocial initiatives.
Specifically, the SocialWorker will provide clinical social work services for persons living with dementia and their caregiving families in the Neurobehavior and Memory Clinic of Northwestern Medical Group; contribute to the recruitment and retention of research participants that meet Center research priorities; manage and facilitate monthly care partner support groups; and participate in the coordination and planning of an annual scientific conference. In addition, they will participate in the preparation and review of annual progress reports and competitive renewal for the NIA and other funding agencies.
The Mesulam Center is a multi-departmental component of the Feinberg School of Medicine. We specialize in the clinical care and scientific study of neurological diseases that interfere with cognition and behavior. Areas of emphasis include age-related memory, language-based and behavioral dementias. We are focused on Alzheimer's disease, Primary Progressive Aphasia and Frontotemporal Dementia. Our three interacting missions are to investigate the causes and clinical characteristics of neurodegenerative diseases, to ensure that our patients and families are the beneficiaries of the latest research advances, and to prepare future scientists and clinicians in our field.
Specific Responsibilities:
* Provide clinical social work services to patients and families of the Northwestern Medicine Neurobehavior and Memory Clinic and center research participants. Complete psychosocial assessments and provide education, counseling and linkage to appropriate community services. Collaborate with multi-disciplinary team of behavioral neurologists, neuropsychologists, psychiatrists, and other clinicians through weekly clinical rounds and patient care discussions.

Our therapists consistently refer clients and their families out of our clinic to meet these needs. Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate. Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities to ensure patient care information is communicated for continuity of care, medical records are complete, and discharge reconciliation is accurate. Actively participates in multi-disciplinary rounds to review changes in patient status, progression and level of care, and discharge plans for all assigned patients to identify resources necessary at discharge and ensure a timely transition, escalating care delays to leadership as appropriate. Communicates with and educates patients and families regarding emotional, social, and financial impacts of illness and mobilizes family/community resources to meet identified needs while advocating for patient and family empowerment in making health care decisions and accessing needed services. Organizes and facilitates patient and family care conferences with the multidisciplinary team. Documents discharge planning evaluation, ongoing assessment, discharge plans, MDRs, barriers to progression of care, avoidable days, and patient and family needs according to standard work. Provides patient and family advocacy, and support patient's choice and patient rights during hospitalization. Communicates with Payors patient's needs for authorization for post-acute care as needed. Other duties as assigned. Assesses readmitted patients for the patient's and family's perceived reasons for the readmission.
