ESSENTIAL JOB FUNCTIONS:
Conducts psychosocial assessments to determine PACE Participants' care needs, preferences, and goals.
Participates as an active member of the IDT for Participant care planning.
Documents accurately and thoroughly in the Participants' electronic health record in accordance with industry and departmental standards including time requirements.
Communicates effectively and openly with other Interdisciplinary Team members on participant status and needs.
Maintains current and thorough written case management records, including periodic reassessments of program Participants.

May develop and lead group counseling, support groups, and/or other relevant activities.

QUALIFICATIONS:
Required:
Master's degree in Social Work from an accredited college or university required.
Current BLS and First Aid required
Preferred:
Current CA licensure as a Licensed Clinical Social Worker (LCSW) preferred
Minimum 2 years of experience in a health-related area preferred
Minimum one year of experience working with a frail and/or older adult population preferred
Valid California Driver's license with good driving history within the last 5 years.
LANGUAGE:

The CRT is co-located on the same campus as the Substance Use Treatment Services (SUTS) program. The program was created as an alternative to and diversion from Emergency Psychiatric Services (EPS), acute psychiatric hospitalization, and incarceration. The CRT will serve a co-ed population with 90% male and 10% female census predicted; all referrals will be persons with justice-involved mental health status. The average length of stay for this program is 14-21 days. The Muriel Wright Recovery Center CRT program is under contract with Santa Clara County Behavioral Health Services Department.

POSITION SUMMARY:
The Social Worker provides direct case management services to On Lok participants. The primary role of the MSW is to assist participants and their family/caregivers to cope with the social and emotional issues and practical arrangements related to maintaining their independence. POSITION SUMMARY:
The Social Worker provides direct case management services to On Lok participants. The primary role of the MSW is to assist participants and their family/caregivers to cope with the social and emotional issues and practical arrangements related to maintaining their independence. With support from the Social Work Lead, the MSW serves as an integral member of the Interdisciplinary team (IDT) providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of participants and their families.

Identify risks and other psychosocial problems.
Identify service needs of each participant; advocate, refer, coordinate and follow up in a timely fashion.

The County of Santa Clara Office of the County Counsel is recruiting a passionate and dedicated attorney to join our Child and Family Protection Team. The Child and Family Protection Team plays a vital role in protecting the welfare of children in Santa Clara County.
