Position Summary
This is a unique hybrid role, working with the CVS Accountable Care Mt. Sinai team. Hours for this position are Monday-Friday . This position will include a hybrid of on-site, community-based, and remote work, including home visits and accompaniments to appointments.
The CVS - MSHP CM social workers are state licensed, and masters level trained and members of the interdisciplinary care team. Using a strengths-based and person-in-environment approach, they utilize motivational interviewing skills to complete comprehensive, clinical assessments of patients that delve into unmet basic needs, social and emotional needs, connectivity with and experience of care, as well as strategies for managing illness/enhancing wellness.
Intervention strategies include appointment accompaniment, community and home visits, collaboration with collateral contacts, and telephonic follow-up. The aim is to address any basic needs (e.g., food insecurity, housing, safety concerns), connectivity to resources (e.g., community supports, public benefits), experience of care (i.e., ensuring meaningful, culturally aligned connection with medical and mental health providers), and experience of illness (i.e. optimization of wellness given complexities of disease). Utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources and optimal, cost-effective outcomes.
Required Qualifications
LMSW or LCSW with New York license. Must reside in New York with local travel in New York City.
1+ years of experience as a licensed social worker 1+ years of experience with Microsoft Office.
1+ years of experience in healthcare/ community-based outreach programs, including experience working with patients with complex chronic illness and/or behavioral health population.
Preferred Qualifications
Exceptional skills in engaging patients/families and utilizing critical thinking skills in assessment and coordination of resources within families and hospital and community networks.
Ability to develop, maintain and monitor connectivity to medical/behavioral health care, safety, and wellbeing of patients.
Exceptional organizational, collaborative, and psychosocial assessment and intervention skills.
Ability to foster a supportive environment for patients and colleagues, and work collaboratively as part of a team.

Bilingual (English/Spanish) preferred.
Case management and discharge planning experience preferred.

The typical pay range for this role is:
$66,575.08 - $142,576.72
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.

Provides educational and goal directed counseling to patients who are seeking transplant.
Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.

Position Overview:
Under the direction of the Supervisor of Prevention Services and in collaboration with colleagues on the treatment team the Social Worker participates in the planning and delivery of evidenced based clinical services and assures fidelity to the particular evidenced based model in terms of service delivery.
Duties & Responsibilities include :
+ Provide evidenced-based clinical services to children and family as delineated and prescribed in the treatment planning process. The clinical components of these services will include completion of the assessment format, negotiation with clients as to the types of intervention that will be deployed, the time and place parameters of the service delivery, schedules for Service Plan Reviews and projected time frame for discharge from services.
+ Provide accurate and thorough documentation of client contacts via the New York State Connections system and with the specific St. Catherine's AWARDS system.
+ Provide specialized therapies including play therapy for children, individual counseling, couple counseling and family counseling as prescribed by the treatment team.
+ Follow Albany County and St. Catherine's requirements for frequency of contacts and for place of contacts as described in the Albany County DCYF and St. Catherine's contract.
+ Clinician will adhere to the CASSP Core Principles, affirming a client's right to self determination and full involvement with the treatment team in terms of developing service goals and consumer friendly methods for attaining these goals; assuring that services are child centered, family focused, culturally competent and community based.
+ Chair Service plan reviews as needed.
+ Maintain regular contact with relevant agency staff and other community agencies providing services to families.
+ This position will have full and complete access to client Protected Health Information (PHI) in order to carry out the duties and responsibilities for this position.

Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
Attend Behavioral Health Center staff meeting twice a month.
Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
Provide referrals and linkages to community supports, resources, and services.
Requirements and Education:
Master's degree in Social Work or Mental Health Counselor.
Experience in human services, case management, line staff at a residential program, or any other related direct care within human services, both pre and post master degree, will be considered.
License and/or Limited Permit eligible, have a limited permit in discipline or Fully Licensed (LMHC/LCSW/LCSW-R), preferred.
Good standing with the NYS Office of the Professions.

**Position Summary**

+ Serves as a member advocate to ensure the member receives all the necessary care allowed under the member's benefit plan and as available through Medicaid benefits and/or other community resources.
+ Develops relationships with hospital social workers and community resources and utilizes available data to assure appropriate care management of catastrophic, acute, and chronically ill members with the goal of appropriate utilization, decreased length of stay, and preventable emergency room utilization.

The typical pay range for this role is:
$51,256.92 - $111,351.24
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
