Summary
The SocialWorker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients. Role:

Actively participates in required meetings to include but not limited to Morning/Weekly Team, Plan of Care, and Family Meetings.
Responsible for attending monthly service plan meetings for participants in assisted living and nursing home facilities.
Provides individual and family supportive counseling at ESP and client's residences and conducts participant and caregiver support groups.
Manages clients during psychiatric hospitalizations.
Assists/advocates for interdisciplinary team, clients and families with community resources.
Works with financial staff to assist with bi-annual financial redeterminations.

Role:
The SocialWorker plans, coordinates and manages social work services for frail older adults and caregivers of Elder Service Plan (ESP) within the community as part of Integrated Disciplinary Team (IDT). Duties include but are not limited to conducting initial, semi, and annual assessments; participating in the Intake process for potential participants; providing supportive counseling to ESP participants and their family members; actively participates in Interdisciplinary Plan of Care meetings and other team meetings.
Responsibilities:
* Participates in the intake screening process in potential participants residence or home as needed, conducts a bio-psychosocial assessment.
* Conducts timely initial, annual, and semiannual psychosocial assessments in client's residences in order to develop participants plan of care to include assessment tools of PHQ-9, MOCA, and CDR.
* Actively participates in required meetings to include but not limited to Morning/Weekly Team, Plan of Care, and Family Meetings.
* Responsible for attending monthly service plan meetings for participants in assisted living and nursing home facilities.
* Provides individual and family supportive counseling at ESP and client's residences and conducts participant and caregiver support groups.
* Manages clients during psychiatric hospitalizations.
* Assists/advocates for interdisciplinary team, clients and families with community resources.
* Works with financial staff to assist with bi-annual financial redeterminations.
* Must be able to travel between Harbor sites and participant's residences as needed in a timely manner.
Requirements:
* MSW from an accredited school of social work
* Current MA LCSW or LICSW License Required, current BLS Certification required
* MoCA certification required or obtained within 3 months of hire
* 2 years of professional experience of which 1 year was in providing direct behavioral health therapy to an adult population
* 1 year of experience working with the frail elder population (paid or volunteer experience), 2 or more years of geriatric experience preferred
* Basic skills for Microsoft Office 365 including Word, Excel, Outlook & Teams
* Experience with dementia care, substance abuse and end of life care preferred
* Critical thinking skills to problem solve, both individually and in a team setting
* Bilingual a plus; English/Spanish, Haitian Creole and/or Portuguese.

Boston Children's Hospital's acclaimed Social Work Emergency Services team responds to social needs and issues in the Emergency Department and throughout inpatient services. We are currently seeking a socialworker for our e/o weekend evening position
Key Responsibilities:
Providing clinical social work services to assist patients and families with personal, family, medical and environmental difficulties.
Performing psychosocial and risk assessments, assigning diagnoses and developing and implementing interventions and treatment plans.
Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan, implementing and following up on treatment and provision of needed services.
Minimum Qualifications
Education:
Master's degree in social work
Experience:
Strong clinical skills and experience with various therapeutic modalities.
Previous medical/hospital social work experience preferred
People from diverse backgrounds encouraged to apply
Spanish language skills are a plus
Licensure/Certifications:
LCSW in the state of MA required, LICSW in the state of MA preferred

The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting.

Clinical SocialWorker for the Adolescent & Young Adult Cancer Program Massachusetts General Hospital
Part Time/Hybrid Position
The licensed clinical socialworker will provide supportive care services for the Adolescent and Young Adult (AYA) Cancer Program. Responsibilities include psychosocial assessment, counseling, group facilitation, and resource navigation for AYA patients and their families.
Following the initial AYA needs assessment, the socialworker will meet with patients and/or families to evaluate psychosocial needs and collaborate with the AYA program manager and nurse practitioner to coordinate care as well as with disease group socialworkers. The role involves providing psychoeducation, mental health support, crisis intervention, counseling, group facilitation, and resource connection to address the wide range of challenges faced by AYA patients.
The ideal candidate will have experience working with adolescents and young adults in an oncology or related medical setting and be skilled in crisis intervention, short-term counseling, bereavement support, trauma, abuse, neglect, and substance use concerns. This is a multifaceted role supporting holistic patient care by integrating psychosocial services throughout the treatment journey.
The socialworker will report to the AYA Program Director and work collaboratively with the AYA program manager and nurse practitioner. Strong clinical, communication, documentation, and teamwork skills are essential.
Qualifications
QUALIFICATIONS:
1. Master's of Social Work (MSW) from an accredited program required.
2. Acute care hospital experience preferred.
