Position Summary:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care; Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy; Education provided to physicians, patients, families, and caregivers; and Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.

Coordination of complex discharge planning. Educates and mentors new social work staff. Acts as resource to all staff members regarding policies and procedures as well as community resources. Works to trend barriers to care as resolve at least one barrier to care.

Works collaboratively with Director of Social Work and Behavioral Health provider to ensure guardianship is up to date. Educates participant regarding health care proxy (HCP). Assists participant in completion of HCP form.
Works collaboratively with fiscal department to maintain participant insurance benefits and completes required documentation of fiscal information in the medical record.
Reviews plan of care with participants, guardian, and/or activated health care proxy as assigned.
Complete authorizations for home care and other approved services timely and accurately.
Completes home and/or skilled nursing facility visits to assess participant as indicated.
Works collaboratively with Palliative care team; Assists with end of life planning as indicated.
Provides timely communication to appropriate staff regarding the following: (disenrollment, conversion to long term care, transfer of sites, participant and/or caregiver demographic changes).
Reports allegations of abuse to appropriate state agency; provides support and resources to participant as he/she will accept; completes required documentation.
Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.
Performs other duties as required.

Patient/family understanding of their disease treatment options
Strengths, available supports, and barriers to care
Determine appropriate level of care including needs for long/short-term skilled-nursing facilities, assisted living facilities, adult day care, etc.
Based on a comprehensive, culturally competent assessment collaborates with the patient and family to create a care plan with measureable goals. Implements short-term interventions to address identified barriers and promote health and well-being.
Provides clinical treatment services in form of triage, crisis intervention, individual or family therapy, screening and planning
Utilize Motivational Interviewing and Coaching skills to assist patients/families in identifying and addressing goals. Using identified strengths, encourage self-determination and independence

The role is hands-on, with both micro-level and macro-level work.
CPEAN provides safe shelter and services for seniors suffering from abuse, neglect or financial exploitation, and works to prevent elder abuse by increasing awareness and providing education. CPEAN operates the first and only emergency shelter program in Massachusetts exclusively for seniors, and is funded largely by philanthropy.
The Supportive Services Specialist is a full-time position. Fully on-site opportunity with primary office location in Roslindale, and expected routine travel to other HSL sites and non-HSL locations across the Greater Boston area.

The Boston Public Health Commission's Homeless Services Bureau (HSB) provides emergency shelter, job training, behavioral health support, and housing services to unhoused individuals in Boston. We serve close to 5,000 individuals every year, and we are one of the largest providers of emergency shelter in New England. We aim to make homelessness in Boston rare, brief, and one time. We do that by problem-solving with new guests at the front door to try to prevent anyone from entering homelessness to begin with. For individuals who do become homeless and use our shelters, we help them quickly move from homelessness to housing by finding a safe place to call home. Once housed, we provide in-home supports to ensure someone does not return to homelessness again. We use a Housing First and racial justice framework, which is built on the foundation that housing is a social determinant of health and a basic need that everyone deserves and does not require sobriety. We believe that everyone, with the right support, can succeed in housing. We foster evidence-based approaches such as trauma-informed care, harm reduction, and motivational interviewing in the delivery of our services, and we ensure services are low-threshold and accessible to our guests.
Duties
Part of a small team responsible for the monitoring, answering, assessing and resolving calls to the Bureau's dedicated Front Door Triage Line to provide information to potential guests and to screen provider referrals for appropriateness.
Collaborates with external social services agencies and acts as liaison to hospitals, mental health facilities and inpatient/outpatient programs, including the maintenance of documentation and follow-up related to guests referred to shelter from these institutions.
Acts as the primary contact for all shelter guests presenting at shelter from institutional settings, as well as interfacing with staff from those institutions.
Collects robust data regarding upstream referrals to shelter to help inform internal and system-wide conversations and policies regarding shelter referrals.
Works to establish regular relationships with staff at referring institutions and assists the Team in developing defined referral procedures.
Acts as the primary liaison in relationships with referring institutions.
Provides education and information to institutions on alternative options to emergency shelter.
Provides clinical assessment, referral and placement services to all new homeless guests entering emergency shelter who report, or present with signs of, mental illness.
Identifies high-risk behaviors in new guests and provides direct crisis intervention and clinical support to the team.
Assertively moves guests on a quick path out of shelter by implementing a diversion plan or making a referral to housing department.
Works in collaboration with the Clinicians of the Behavioral Health Department, to ensure that clients presenting with acute mental health symptoms are assessed for the most appropriate services and/or referral.
Assists guests with DMH eligibility applications and, in collaboration with the Behavioral Health Department, coordinates DMH services for clients including outreach services from the Homeless Outreach Team of DMH.
Works in collaboration with Recovery Services Bureau for placement into substance abuse and dual-diagnosis treatment if needed.
Develops and maintains referral sources for homeless guests by establishing contacts and working in collaboration with area social services agencies.
Maintains up to date and accurate progress notes and records for the department, including HMIS.
Assists in compiling records and statistical reports.
Attends assigned meetings and trainings.

Role:
The Social Worker plans, coordinates and manages social work services for frail older adults and caregivers of Elder Service Plan (ESP) within the community as part of Integrated Disciplinary Team (IDT). Duties include but are not limited to conducting initial, semi, and annual assessments; participating in the Intake process for potential participants; providing supportive counseling to ESP participants and their family members; actively participates in Interdisciplinary Plan of Care meetings and other team meetings.
Responsibilities:
Participates in the intake screening process in potential participants residence or home as needed, conducts a bio-psychosocial assessment.
Conducts timely initial, annual, and semiannual psychosocial assessments in client's residences in order to develop participants plan of care to include assessment tools of PHQ-9, MOCA, and CDR.
Actively participates in required meetings to include but not limited to Morning/Weekly Team, Plan of Care, and Family Meetings.
Responsible for attending monthly service plan meetings for participants in assisted living and nursing home facilities.
Provides individual and family supportive counseling at ESP and client's residences and conducts participant and caregiver support groups.
Manages clients during psychiatric hospitalizations.
Assists/advocates for interdisciplinary team, clients and families with community resources.
Works with financial staff to assist with bi-annual financial redeterminations.
Must be able to travel between Harbor sites and participant's residences as needed in a timely manner.

MSW from an accredited school of social work
Current MA LCSW or LICSW License Required, current BLS Certification required
MoCA certification required or obtained within 3 months of hire
2 years of professional experience of which 1 year was in providing direct behavioral health therapy to an adult population
1 year of experience working with the frail elder population (paid or volunteer experience), 2 or more years of geriatric experience preferred
Basic skills for Microsoft Office 365 including Word, Excel, Outlook & Teams
Experience with dementia care, substance abuse and end of life care preferred
