ESSENTIAL FUNCTIONS:
Serve as a case manager or co-writer with the teacher of all student assessments and learning plans that focus on educational needs, as well as other needs including IEP goals/accommodations as applicable that may affect a students functioning in school. Optimal Care Hospice Medical SocialWorkers are trained professional SocialWorkers who assist patients and families living with terminal illnesses. Required Education and Experience:
Master's degree in social work.

What Will you Do?
