The Life Services Life Coach provides one-on-one coaching and development for participants in the Life Services program as defined by member needs both urgent, immediate, and long term to prepare members for employment or education programs.
Essential Functions:
Complete Life Services needs assessment by gathering information from the member, the member's family, stakeholders, and other parties Support a positive environment for the member by motivating, recognizing and engaging participants through proper communication and individualized practices Maintain consistent documentation on Life Services participants including but not limited to: training records, Life Learning Plan development and status updates, agency contacts, employment/interviewing activities and life service needs Research developments in the fields of workforce and career development for those in and working to get out of poverty Act as a liaison to social service agencies, government agencies, employers, employment programs & education programs and advocate for Life Services participants Documents all sessions and interactions in the appropriate databases in a timely manner Coordinate logistics for appointments as necessary Support development of Life Learning Plan and monitoring and reporting on participant status Arrange and coordinate services with community leaders; partners to maximize utilization of available resources and support groups Facilitate individualized service plans based on member needs Regular travel to conduct member, provider and community based visits as needed to ensure effective administration of the program Perform any other job duties as requested

Education and Experience:

Licensure and Certification:

CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 - March 31) as a condition of continued employment. Employees hired during Influenza season will have forty-five (45) days from their hire date to complete the required vaccination. CareSource requires all employees to be fully vaccinated (two weeks removed from completion of a two-dose mRNA series or two weeks removed from a one dose vaccine) against the COVID-19 virus. Employees are required to disclose and provide proof of their vaccination status as a condition of continued employment. Candidates who accept an offer of employment must upload proof of vaccination prior to their start date. Failure to meet the vaccination requirement, including providing proof of vaccination prior to the start date, may result in rescission of an employment offer or termination of employment. Provides individualized support to consumers to identify barriers, create community participation and develop methods for accessing activities and services in community settings. Essential Functions

9. Implement behavioral intervention plan when needed to assure consumer safety and promote learning. Coordinates consumer programming with Center's activities, services, and staff.

11. Acts as a mandated abuse reporter.

Qualifications Required
4 year degree preferred, plus one year of related experience working with people with disabilities, or an approved combination of experience and education.
Possession of a valid California driver's license and clear driving record.
CPR/First-aid Certification (Hope can provide the training, if needed)

Required Knowledge and Skills
1. A belief that all people are valued members of the community.
2. An understanding of the principles of normalization and commitment to community integration.
3. Ability to work independently and cooperatively as a member of the team.
4. Excellent communication and documentation skills including use of computerized word program and data base.
