TITLE: HOUSING COORDINATOR
GENERAL DESCRIPTION:
Manage service and care provision for residents in Congregate and Supportive Housing sites.
Activities include recruitment and assessment of applicants, community outreach, service planning and coordination, provision of direct individual and group services, development of congregate "community", participation in multidisciplinary teams.
QUALIFICATIONS:
The position requires an individual who is a self-starter with a positive and enthusiastic attitude as well as excellent oral and written communication skills. Must be able to work independently as well as part of an interdisciplinary team and have the ability to problem solve and provide resource and referral options to residents. Computer proficiency required. Knowledge of housing issues, service delivery systems for elders and persons with disabilities, and understanding of the aging process and disability issues, ability to establish and maintain trust and working relationships with consumers, families, staff, housing and other community agencies. Bachelor's Degree in a Human Service related field with a minimum of two years experience working with elders, individuals with disabilities or caregivers preferred.
Requires: valid driver's license and insured automobile in good working condition; ability to visit with consumers in their own home, availability to work between the hours of 9:00 a.m. to 5:00 p.m., availability to work in the event of an emergency during off hours.
DUTIES & RESPONSIBILITIES:
1. Recruit applicants for congregate housing through community education and outreach.
2. Facilitate Multi-disciplinary Assessment Team (MAT) as required
a. Tenant selection procedures;
b. Tenant application form;
c. Tenant termination process.
3. Develop and implement a supportive service plan for each resident including:
a. Identifying the supportive service needs and resources of each tenant;
B. Linking each tenant with appropriate community resources;
C. Reassessing the supportive service needs of tenants and revise the supportive
Service plan as needed.
4. Coordinate the delivery of supportive services to residents of congregate housing.
5. Facilitate open and ongoing communication, problem-solving and conflict resolution among
residents.
6. Integrate the residents into the community by:
a. Alerting residents to activities and events in the community;
b. Encouraging the use of the community space (for example, mealsite, senior
center) in the congregate housing facility by others;
c. Providing information to the public about congregate and supportive housing.
7. Other duties as assigned by immediate supervisor or member of Management Team.
SUPERVISION:
The Housing Coordinator reports to the Housing/Home Sharing Program Director.

The pay for the position is 32.96 for LPN's and 36.04 for RN's.

ESSENTIAL FUNCTIONS - DUTIES & RESPONSIBILITIES:
1. Manage all aspects of assigned community programs, including day-to-day activities and volunteer intakes and supervision meetings along with implementing volunteer reference checks, orientation, annual training, and on-going recruitment activities.

GENERAL DESCRIPTION:
The PCA program assists people of all ages in Franklin, Hampshire, Hampden, and Worcester County. For this position, we are able to offer FT or PT (at least 20 hours per week or FT at 35 hours per week). We expect that these positions will be available for a 9-12 month period. This position is designed to complement LifePath's approach to meeting the needs of LifePath Home Care consumers and older adults in Franklin County and the North Quabbin Area. This position follows an evidence-based model of volunteer training and implementation where volunteers are matched one-on-one with consumers. Priority responsibility is to manage and support the daily operations of LifePath community-based programs, volunteer recruitment, management & stewardship; and participating & coordinating LifePath outreach activities. Current LifePath community programs supported by this role encompass assisted transportation to medical appointments, phone pals companionship, a social support network for LGBTQIA+ older adults and other community projects that may evolve. This position has a cross-department administrative support component focused on effective integration of programs and reporting & monitoring systems, and providing back-up support to the Director of Community & Volunteer Resources, as requested.
ESSENTIAL FUNCTIONS - DUTIES & RESPONSIBILITIES:
1. Manage all aspects of assigned community programs, including day-to-day activities and volunteer intakes and supervision meetings along with implementing volunteer reference checks, orientation, annual training, and on-going recruitment activities.
