    We are currently seeking a full-time physical therapist for our outpatient and multispecialty clinics in our Boston location Key Responsibilities: Performing complex diagnostic evaluations upon referral and by departmental and professional guidelines and standards. Reviewing patient medical history/record and selecting, administering, and interpreting standardized and non-standardized tests and measurements to assess patients' functional status related to cardiopulmonary, developmental, musculoskeletal, neurological, and sensorimotor function. Integrating knowledge into clinical decisions and recommendations. Developing and implementing individualized patient treatment programs. Establishing treatment goals and administering appropriate physical therapy procedures and modalities. Preparing discharge plans and home treatment programs. Providing education to patients and families. Monitoring treatment outcomes, modifying programs as needed, and communicating recommendations to physicians and other providers. Recommending orthotics, adaptive/assistive equipment, and technology for patients with complex, rare, or multiple dysfunctions. Integrates knowledge into clinical decision-making to meet specific treatment objectives. Liaison with equipment designers and vendors regarding equipment specifications and patient needs. Training patients, families, and caretakers in the use/operation of prescribed orthotics/equipment, monitors and evaluates effectiveness, and initiates modifications as needed. Recording assessments, diagnoses, and treatment inpatient medical records according to established documentation standards. Prepares reports, correspondence, referrals, and other materials for schools, providers, payers, and others as needed. Identifying and developing clinical knowledge and skills through participation in professional organizations, attending continuing education programs, peer review, and other related internal and external resources. Serving as a clinical instructor in a departmental student externship program upon completing eligibility requirements. Planning learning objectives and arranges related clinical experiences. Monitoring performance, providing guidance and feedback, and preparing and conducting student evaluations. Acts as a supervisor for assigned direct reports, including physical therapist I, physical therapist assistants and rehab aides. Assists in monitoring work assignments, identifying areas for improvement and professional development, providing instruction and feedback, and participating in performance evaluations. May participate in departmental training and staff development programs. May supervise assigned direct reports, including physical therapist assistants (PTA) or rehab aides. Minimum Qualifications Education: Bachelor's degree in physical therapy Experience: Minimum of 3-5 of licensed PT experience required Previous hospital-based outpatient PT experience highly preferred Licensure/Certifications: Physical Therapist license in the state of MA New hires are eligible for a $5,000 sign on bonus! Essential Functions * Collaborate with interdisciplinary care teams (ICT)-including LTSC, GSSC, primary care providers, specialists, and behavioral health treatment facilities-to support program enhancements, process improvements, care coordination, discharge planning, and aftercare. * Participate actively in ICT meetings, contributing insights on behavioral health concerns, treatment options, care strategies, and engaging in collaborative decision-making regarding enrollees' care. * Receive and triage behavioral health and substance use referrals, coordinating with the ICT and primary care providers to refer enrollees to appropriate inpatient or outpatient services. * Develop, update, and implement enrollee-centered care plans in collaboration with enrollees and care teams, promoting self-care, shared decision-making, and addressing identified needs through routine engagement and proactive outreach. * Conduct outreach, assessments, and home visits via telephonic, electronic, or in-person contact for identified enrollees, including routine evaluations and post-transition care. * Monitor clinical status to identify early signs of deterioration, intervening proactively with the ICT to prevent unnecessary hospitalizations and escalate behavioral health needs during weekly case conferences. * Establish consistent communication and reporting schedules with providers and enrollees to review enrollee status, progress toward goals, care needs, utilization plans, and follow-up. * Ensure compliance with DSNP regulations and quality assurance standards by documenting, monitoring, and evaluating enrollee interventions and care plans accurately within electronic medical record systems. * Serve as a key resource and liaison within the ICT by addressing enrollee questions, concerns, and care needs, facilitating access to services, and performing additional duties as assigned by the supervisor. Behavioral Health Specialist Supervisor is a key component of the interdisciplinary team that provide support services for patients with Substance Use Disorders. The role of the BHS supervisor is to support behavioral health specialist staff and the program leadership to ensure that the program is operating at its best. BHS supervisors work closely with nursing to provide evidence-based care to our patients with SUD. The BHS supervisor will perform functions efficiently and thoroughly to assure that the patients are safe during the detox process. Schedule varies. Days evenings and weekends will be required. You have experience in a medical or office setting and are comfortable interacting with patients, as well as managing administrative responsibilities. Our Behavioral Health Services programs provide residential and outpatient treatment for those struggling with mental illness, developmental disabilities, or substance use disorders. For many individuals, our programs literally save lives. Behavioral Health Specialists are a key component of the interdisciplinary team that provide support services for patients with Substance Use Disorders. The role of the BHS is to monitor clients during the detox process and in early recovery. BHS staff will work closely with nursing to provide evidence-based care to our patients with SUD. BHS s will perform functions efficiently and thoroughly to assure that the patients are safe during the detox process. BHS staff will assist that the staff are supported during the admission process, day to day clinical activities and discharge. Schedule varies. Days evenings and weekends will be required. Essential Functions Relates to clients and colleagues in a compassionate, professional, and respectful manner as measured by observed interactions. Assures safety and support to all clients admitted to The Dimock Center. Able to assess, communicate effectively and maintain therapeutic boundaries through setting limits and enforcing structure and rules of the program in a respectful manner. Supports all milieu staff in all aspects of the program including: safety checks, client and room searches, supporting client s admission and discharge processes. Participates in creating a welcoming and supportive environment for new clients in the program. Recognizes the importance of individual and cultural differences that influence patient behavior and applies this understanding to the care of patients. Participates in the patient admission process; provides explanation of Dimock paperwork and completes all necessary intake documentation. Conduct property search of clients belongings on admission and discharge. Observes and reports to Nurses clients who appear sick, agitated or depressed. 10. Uses negotiating skills to minimize and de-escalate potentially dangerous situations. 11. Supervises and supports clients as needed throughout the program day. 12. Assists new clients who enter the program with paperwork, orientation and settling in. 13. Attends to the overall operation and management of the living environment. 14. Ensures that the living space is safe. 15. Facilitates groups about Recovery and Addiction process. 16. Establishes and maintains professional boundaries for clients to ensure client safety. 17. Runs the basic day-to-day functions of the program to ensure that the unit is safe, that the clients receive meals three times a day and assist clients with any special needs. 18. Attends and participates in all staff meetings and attend trainings. Assists sick or disabled clients with personal needs when they are unable to perform these tasks by themselves. 20. Oversees and ensures patients follow all Dimock Policies. 21. Ensures that the program environment is safe and conducive to healing and recovery. 22. Works with program staff to develop and support new programming. 23. Other duties as assigned. Competencies Exhibits professional demeanor in tone of voice, appearance, and movement. Work Environment 24 Hour Acute Treatment Setting. Physical Demands Requires lifting and frequent carrying of up to 20 pounds. Position Type/Expected Hours of Work Schedule varies but is typically Tuesday to Saturday OR Sunday to Thursday. Days evenings and weekends will be required. The following shifts are typically available for full time roles. 7am - 3pm 3pm - 11pm Required Education and Experience Bachelor s Degree in related field is preferred. Significant experience working with those suffering from Substance Use Disorder, or other human services work strongly preferred.
