How you'll contribute
A Case Manager who excels in this role:
Completes departmental orientation, initial and annual competencies.

Qualifications and requirements:
Bachelor's or Master's degree in Social Work, or as required by state regulations (verify state-specific requirements).

Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.

How you'll contribute
The Environmental Services Aide cleans and services building areas including patient rooms, common areas, floors, office areas etc. Moves furniture, trash, equipment and supplies and performs a variety of environmental services duties to maintain the hospital in a neat, safe and sanitary condition.
Qualifications and requirements
High School Graduate or equivalent preferred.
Previous experience in hospital setting preferred.

Essential Functions:

How you'll contribute
The Clinical Dietitian plans and delivers nutritional care to patients according to the physicians' diagnoses. Performs as a member of the multidisciplinary care team. Screens newly admitted patients to identify those at nutritional risk.
Qualifications and requirements
Bachelor's degree in course work approved by the Commission on Accreditation for Dietetics Education (CADE) of the American Dietetic Association (ADA) or a post Bachelor's Certificate in Dietetics.

What you will do in this role:
Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patient
Performs patient care considering needs specific to the standard of care for patient's age; includes geriatric and general inpatient rehabilitation patients
Under the supervision of the R.N., assists with the assessment on all patients and reassessments as per policy. Identifies and initiates appropriate rehabilitation nursing interventions
Under the supervision of the R.N., performs timely and accurate QI assessments
Carry out the plan of care as indicated by the patient's needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient's condition and individual needs
Interacts professionally with patient/family and involves patient/family in the formation of the plan of care
Demonstrates ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management

Qualifications - External
Graduate of accredited licensed practical/ vocational nurse program
Current state licensure as Licensed Practical/Vocational Nurse
CPR/BCLS certification. ACLS preferred

Obtains and records temperature, pulse, respirations, blood pressure, weight, intake and output.
Makes rounds with oncoming shift to ensure the unit is left in good condition.
Communicates appropriately and clearly to nurse, supervisor, co-workers, physicians and the Rehabilitation Services staff..
Response to the patient is appropriate in emergency or physically distressful situation.
Performs other duties as assigned within the nurse assistant's scope of practice.

How you'll contribute
A Physical Therapist who excels in this role:
Assumes responsibility and accountability for the care of those patients being treated by Physical Therapist Assistants and Therapy techs. May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual.
