DUTIES:
Observing Residents
Reporting any health issues to the supervising nurse
Taking care of a Resident's personal hygiene, including bed bath, shaving, etc.
Setting up of meal trays, and documenting food/fluid intake
Feeding Residents & serving nutritional supplements
Making beds & keeping the Residents' space clean and tidy
Transporting Residents within the Facility
Turning bedridden residents to prevent bedsores
Maintaining Confidentiality of all Resident & Facility data
REQUIREMENTS:
Must be able to work as a team member
Successful completion of a CNA program
Current CNA State Certification
Must be in good standing with State Registry
About us:
Warren Center is an eighty-bed skilled nursing facility located at 42 Gurney Lane. Our facility provides a wide spectrum of nursing care to the residents of Warren County. We provide short-term rehabilitation services to people who cannot immediately return to their homes because of an accident or illness. We also provide long-term care for individuals who are unable to care for themselves. We look at each person from an individual perspective. We then utilize a team approach working with the residents and their families to develop a unique plan of care to meet their needs. Meet with individuals and families face to face in the community and their homes

ESSENTIAL JOB FUNCTIONS
The following duties are mandatory requirements of the job:

Visits multiple units as required and reports all incidents to the Operations Manager.
OTHER RESPONSIBILITIES
Ability to work effectively with homeless families with a family member who may be diagnosed with mental illness or substance abuse disorder.
Crisis management and de-escalation skills.
Crib checks are done daily on the 7am-3pm and 3pm-11pm shift.
Wellness checks are conducted on each shift: 7am-3pm, 3pm-11pm & 11pm-7am.
EDUCATION AND CERTIFICATION
The minimum requirement for this position is a High School Diploma or Equivalency (GED).
Valid NYS Security Guard License required.
Criminal background checks are required.
NYS Mandate Reporter Certification.
Valid Safety Coordinator (F80) required.
Fire Guard (FO2) required.
Basic First Aid (required)
Driver's License preferred.
SPECIAL SKILLS AND KNOWLEDGE
Knowledge of safety practices and standards of the organization.
Experience working with diverse populations.

Facilitate onsite wellness events as assigned by Wellness Coordinator to include completion of group note and sign in sheet.
Work with assigned ESSHI tenant on skill building identified on individualized service plan such as computer literacy, kitchen and cooking safety, healthy eating and meal preparation. Resident Aide will document training and skills building provided to the tenant and submit summary to assigned case manager.

Under general supervision, the Nuclear Medicine Technologist performs standard imaging procedures using radioactive materials to assist in the diagnosis and treatment of disease. This role also involves maintaining accurate records, ensuring radiation safety protocols, and providing guidance or training to other staff members as needed.
Responsibilities:
Perform routine nuclear medicine diagnostic procedures following physician orders and safety protocols
Prepare, calculate, and administer radiopharmaceuticals
Operate nuclear medicine imaging equipment including Siemens EVOSPECT and Phillips Forte cameras
Utilize NMIS in the hot lab for data entry and quality control
Access and document patient information in EPIC and Synapse PACS
Maintain detailed records of imaging results, procedures, and radiation doses
Ensure proper handling, storage, and disposal of radioactive materials
Support quality assurance and safety procedures in compliance with state and federal regulations
Assist in training and mentoring new staff as assigned
Requirements:
Current New York State Nuclear Medicine Technologist license/registration
ARRT (N) or CNMT certification in Nuclear Medicine, or board-eligible
CPR certification
