This position reports to a Case Management Supervisor. Duties and Responsibilities: Coordinates and provides safe, timely, effective, efficient, equitable, culturally sensitive, and client- centered care. For SUD Clients: Handles case assignments, assesses barriers to recovery in the various life domains, may draft SUD case management-specific service plan, review progress in case management goals. For MH Clients: Handles case assignments, assesses barriers to recovery in the various life domains, coordinates mental health treatment planning including goals for CPST services, provides input, and works collaboratively with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan. Is familiar with the diagnostic assessment completed for the clients served, which includes a review of diagnosis, identification of current symptoms, risk, functioning, identified needs in the client's life domains shown as Z-Codes to determine eligibility for Case Management or CPST services and the level of care needed. Works collaboratively following up with observations with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan. Adheres to Lighthouse's protocol for appropriate "care management" activities as part of the continuum of services to meet each client's defined needs and as defined in the treatment plan. Provides follow-up and appropriate referrals to community resources are essential. Collaboration and coordination of services to ensure continuity of care and efficiency of services across systems is vital to early recovery. Provides linkage and assistance in accessing inpatient or more restrictive treatment, as medically necessary. Stays abreast of best practices in modalities of intervention to maintain high-quality care management that is medically necessary to client specific treatment plan. Evaluates daily living functioning and assist clients in creating goals to improve daily living. Completes billing and documentation accurately in a timely manner. Sees clients weekly or as clinically appropriate according to the client's level of care. Ensures all community activities comply with HIPAA, 42 CFR Part 2, and Mental Health laws and regulations on confidentiality of PHI. Keeps abreast of community resources and makes needed referrals to meet client needs on a timely basis. Coordinates and collaborates with other community agencies regarding client's treatment to promote positive treatment outcomes. Participate in community collaboration meetings. Advocates and raises awareness on behalf of clients for community and local services to meet client needs. Required Qualifications: CDCA 2 years relevant experience in case management, social services, or a related field Current State of Ohio Driver's License and active insurance Must pass BCI check, all Corporate Compliance checks, and employment drug screen Desired Qualifications: Knowledge of Job and Family Services procedures Bachelor's Degree in Social Work, Psychology, Counseling, or a directly related human service field. Deep River is a comprehensive substance abuse disorder treatment facility in Neillsville, WI committed to helping residents move into sustainable recovery. Services include a full range of DHS 75 and DHS 83 license requirements and include a trauma informed approach, with gender specific services for men. Our beautiful facility provides an environment that, combined with the passion for the SUD field of our team members, allows residents to experience meaningful change and hope. This is a part-time role with hours on Monday-Friday from 1pm-5pm. Under direction of program and clinical supervisor, performs core functions of a Case Manager. ESSENTIAL DUTIES AND RESPONSIBILITIES: This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time. Markets services and/or receives referrals of customers with a variety of needs which require the application of professional social work counseling skills to solve. Conducts assessments of customer needs through interviews, collateral contacts and other investigations, develops a casework plan with customers, and performs agreed upon and necessary casework services. Provides case management services, and coordinates the various social, community, and mental health services received by customers. Establishes and maintains effective public and working relationships and collateral contacts. Maintains detailed case records, develops and prepares statistical reports, makes recommendations concerning case disposition, and testifies at hearings as required. Attends and participates in staff development programs, including in-service training, staff meetings, and professional seminars. May provide consultation to other social work staff and para-professionals. May serve as a representative of the agency to interested individuals, professionals, and community groups. May administer specialized programs within a community or the agency. Participates as a member of a quality team and interacts with primary, external, and internal customers of the agency in such a way that the customer experiences increasing quality. Performs other duties as required.     Purpose: Care Management is a service that helps children with chronic physical or emotional issues obtain and utilize the medical, social and community services they need to get and stay healthy. Care Coordinators learn all they can about the child and family's unique needs. They work with other care providers to develop a plan of care and help members determine and take the actions needed to make and keep appointments, adhere to medications, obtain educational and health advocacy, and access benefits. Position Overview: The Social Work Supervisor will not only oversee the coordination of services for children with chronic medical and/or behavioral health conditions, but also provide hands-on support to the families we serve. The role involves converting cases from Outreach to Enrollment and managing a small caseload of children with high-risk behaviors. The Social Work Supervisor role is pivotal in connecting children and families to the services needed to stay as healthy as possible and avoid unnecessary trips to the emergency room. In collaboration with the care coordination team, the Social Work Supervisor will assist with completing risks and needs assessments, developing person-centered care plans, scheduling, and tracking appointments, educating members, and coordinating other aspects of members' health and community services. The role will require some evening availability and on-call coverage. As this is an evolving program, additional responsibilities will be added. KEY ESSENTIAL FUNCTIONS: Provides regularly scheduled weekly supervision to care management staff members of the NYC team, and Staten Island as needed. Provides administrative oversight to ensure compliance with TJB, DOH, and CBC standards. Monitors service provision, referrals, and census data to maintain program effectiveness.
