Essential Functions: Maintains all inventory records for capital and minor equipment assets; communicates, coordinates and reconciles information with Tribe's inventory tracking system. Verifies proper classification of fixed high risk assets. Assigns inventory barcode tags to all capital and minor equipment assets. Responsible for completing a physical inventory of Capital equipment on a bi-annual basis including identifying equipment, tagging and updating inventory database. Maintains warehousing and surplus equipment inventory and records. Coordinates the distribution and redistribution of equipment; maintains historical files of all Tribal equipment. Reconcile inventory system for the removal of equipment from inventory; and Coordinates distributions and disposals as needed. Reports the capitalized equipment/asset financial information to the Finance Department. Insures Federal and State grant property management compliance. Identifies, researches and solves equipment inventory discrepancies. Coordinates removal and moving of tribal property as requested by Tribal departments. Provides back up support to department staff in their absence or as needed. Generates a variety of reports as requested. Performs a variety of other duties as assigned by supervisor. Education, Experience, and Licenses: Requires a high school diploma or GED. College course work in inventory management and computer training preferred. Relative procurement experience to include inventory management experience or an equivalent combination of education, training, and/or related work experience preferred. Must maintain a valid Oregon driver's license or obtain one upon hire, qualify for the Tribe's vehicle insurance, and provide proof of personal vehicle insurance. Qualifications, Registrations, and Certifications: Requires ability to efficiency plan, schedule and accomplish a variety of activities and projects simultaneously and within appropriate time constraints. Ability to establish and maintain effective work relationships in meeting and dealing with others. Ability to use computer application software packages used for spreadsheets, databases and word processing applications. Requires knowledge of inventory control and inventory control computerized database system preferred. Ability to be able to make decisions under pressure and time constraints. Ability to make some decisions related to work order requests and follows standard practices, policies and procedures of the Tribe with minimal supervision. Knowledge of and sensitivity to Native American culture required. Environmental Factors and Conditions/Physical Requirements: Continually requires the ability to express or receive detailed information or important instructions through oral communication. Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers. Often requires walking or moving about to accomplish tasks. Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally. Occasionally requires lifting of up to 50 pounds. Due to the nature of the interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations. BHN will provide practical support services to patients with complex medical and mental health conditions, frail patients, and/or patients facing homelessness or other stressors. Main function of this position is to facilitate patient's access to services that will increase better health outcomes. BHN will care coordinate for primary care patients and will assist in assessing the treatment needs of patients, determines the appropriate level of care and coordinates complex demographic, insurance and behavioral health clinical information. Identifies behavioral health services requiring insurance verification and initial insurance authorization for treatment and ensures required authorizations are in place. Communicates and coordinates all pertinent collateral clinical information to the specialist and facilitates the communication/coordination between, Behavioral Health Consultant (BHC), specialist and Primary Care Provider (PCP). Assisting Behavioral Health Consultants with scheduling patients Administrative duties including but not limited to: answering phones, attending to voicemails, triaging patients, scheduling, and entering health information in the electronic medical record Assist Behavioral Health Consultants in care planning and follow up with patients Responsible for identifying general social determinants of health resources in the community, maintain a database, and assist patients in connecting with resources. Maintain data on number and types of referrals and will gather and maintain data on successful referrals. Reliable attendance and presence at work. Education and Experience Required : Entry level High School Diploma or GED equivalent Experience in medical setting preferred Strong understanding of cultural competency with patient population Computer literacy and familiarity with MS Office software Exposure and comfort working in medical setting, with medical and psychiatric issues Knowledge, Skills and Abilities Required: Commitment to the mission of care coordination and de-stigmatizing and promoting access to BH services Passionate, trustworthy, and empathetic when working with clients Ability to build relationships with different types of people, including clients, organization members, and health care providers Good communication and interpersonal skills and ability to speak concisely to clients and team members Organized with confidential client material and appointment tracking Flexible and adaptable in response to changing client and health care providers' needs Interest in working with patients with chronic health problems and behavioral health needs Praxis Health is a family of medical groups providing high quality healthcare throughout the state of Oregon.
