Responsible for delivery of quality primary dental care to the patients of Little River Medical Center. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Shall diagnose and/or treat any diseases or lesions or conditions of the oral cavity and associated adjacent structures. Administer or prescribe drugs or therapy utilized in the treatment of dental or oral diseases. Shall use X-ray for dental diagnostic purposes. Shall administer local anesthetics for dental procedures. Direct and manage the supportive dental personnel on a day-to-day basis. Supervise clinical components of services at his/her site. Liaison with dental care resources in proximate secondary dental care resources. Consultant for dental in-service training of all staff. Support and carry out the philosophy of the Center. Serves as a resource person for other agencies in activities related to health care or to clinical operation. Coordinate clinical activities with those offered by other agencies. Develop and maintain clinical skills by seeking consultation from appropriate resources. Must maintain their license to practice dentistry in the state. Supervises the day-to-day activities of his/her office staff, including dental assistants, hygienist, and customer service representative. The CHW Supervisor will effectively communicate feedback and provide ongoing coaching and mentoring to staff while supporting a learning environment to advance team skills and promote professional development. Besides providing supervision to the other CHWs, the CHW Supervisor will help individuals navigate both the health and human services system; provide culturally appropriate health education; help build community capacity (for more resources), provide needed follow-up and help identify gaps in services within the community. The CHW Supervisor will report directly to the Executive Director. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Community Health Worker Supervisor will: Act as a subject matter expert on CHW program workflow and care model, providing leadership, guidance, and best practice techniques to CHW staff. Carry a personal patient caseload. Conduct social determinants of health interviews with patients to identify needs and document appropriately. Develop a plan to address moderate and high social determinants of health and health equity needs. Assist patients with problem-solving barriers to health stabilization by identifying, locating, connecting to, and navigating needed community and medical system services. This may include accompanying patients to appointments and assisting patients with completing forms to access needed services. Link clients to and inform them of available community resources. Teach clients the knowledge and skills needed to obtain care (empowerment). Actively build individual/client, community, and team capacity. Be a spokesperson and advocate for clients when they are unable to speak for themselves. Serve as a lead community representative by attending community coalition and workgroup meetings, making presentations on behalf of the programs and fostering collaborative relationships with community partners, community resources, health plans, and providers. Leadership and Human Resources Management: Promote growth of Community Health Worker team members in areas of patient-centered care, education, and cross organizational care. Provide continual coaching support and guidance in promoting adherence to the CHW Care Model and other evidence-based practices. Lead onboarding, orientation, and training of CHW staff. Assign incoming patient referrals and monitor volume of referrals assigned to CHW staff. Ensure employees have the needed tools and resources to achieve goals and ensure compliance. Work closely with Program Director on day-to-day program operations. Documents all contacts with assigned patients in a timely manner in established EMR. Gathers, prepares and reports on community engagement, community resources, gaps in services and community needs to leadership. EDUCATION and/or EXPERIENCE: Bachelor's degree in social work, Human Services, Healthcare, or related field. Four (4) years of relevant community health worker, community outreach, or health education experience may be considered in lieu of degree. Must be familiar with Horry County and have “lived” experience in the area. Prefer someone who is already connected to the community through prior work experience and/or volunteer work, involvement with charitable organizations, faith-based communities, hobby groups, etc. Two (2) years of experience in a supervisory role. Previous experience in a healthcare, social services and/or non-profit setting. Prefer someone with experience in community outreach, health education, or in a role representing or supporting local community agencies. Strong people and communication skills, both verbally and in writing. Must have a valid driver's license and provide own transportation. Ability to speak Spanish preferred, but not required.     Behavioral Health Counselor The Behavioral Health Counselor (Provider) provides day-to-day management and care to the patients of the Center. The Provider is responsible to the Behavioral Health Director, Chief Operations Officer, and Chief Executive Officer for exercising each of several functions described in this position description and is accountable, through the Director, CEO and COO for the delivery and quality of the behavioral health services provided to patients at the Center and its satellites. The Provider will serve as an important part of the management team. WHY LRMC: Little River Medical Center is a non-profit community health center within Horry County. At Little River Medical Center, we strive to offer exceptional health services and deliver quality, compassionate care to everyone. We provide a wide range of affordable health and support services for every family. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Administrative Responsibilities: Serve as Provider for the Center and its satellites. Assists with the establishment of behavioral health care standards and protocols for each service described in the Center's health care plan and to ensure quality of service; to assist with development of the Center's behavioral health care plan with the assistance of staff to ensure that all goals and objectives are outlined. Be responsible for the day-to-day management of the patients seen in a manner that ensures effective and efficient performance. Operate within the parameters of a written position description that complies with the Center's policies, rules, and regulations. Establishes appropriate systems to assure quality of patient care. See that the Quality Assurance Protocols, HIPAA, Corporate Compliance and CLIA standards are adhered to. Establishes appropriate systems to assure the quality of patient care. Maintains all Federal initiatives pertinent to patient care and participates in the process of budget development and in financial management decisions as they affect clinical activities. Participates regularly in administrative meetings as a member of the Center's management team. Clinical Management Functions: Manages individual patient care activities at the Center. Confers with the Director, COO and CEO as needed. Supports and maintains the competence and quality of all clinical support staff. Consults with members of the provider staff on clinical issues or coordinates appropriate continuing education and in-service training. Assures safety and adequate maintenance of Center's medical records. Conducts or supervises an ongoing quality assurance program. Clinical Responsibilities: Provide behavioral health care service to patients at the Center as scheduled. Provide behavioral direction for health care activities. Periodically review patient health care records for accuracy and completeness. Provide counseling orders and patient referrals as appropriate. The provider shall refer and coordinate in-patient hospitalization for the Center's patients at the hospital of the patient's choice and/or at a facility as required pursuant to insurance contact guidelines. Professional Liaison Functions: Establish links with public and private clinical referral sources. Assumes an active membership role in national, state, and local behavioral health care societies. Conducts or coordinates activities designated to promote the community's awareness of the Center's health care program. Conducts or coordinates activities designed to mobilize community resources behind health promotion. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. EDUCATION and/or EXPERIENCE: Master of Social Work (MSW), Licensed Clinical Social Worker (LCSW), or Licensed Independent Social Worker - Clinical Provider (LISW-CP) preferred. Has certification and licensure by the South Carolina Board of Social Work Examiners. Be “public health” oriented. Be full time Center employee. CERTIFICATES, LICENSES, REGISTRATION Have a valid LISW, LISW-CP, LCSW, or LMSW license to practice behavioral health counseling in the state of South Carolina.
