Job Description Our Hospice is looking for Home Health Aides (HHA) for our home side patients. This person works with a caseload of patients alongside a RN Case Manager. Our patients and families are in the most important time of their life and need non-skilled nursing to help navigate their pathway. In this position, every day is a little different and you are on the go visiting patients in your community. This role will work Monday-Friday, no holidays or weekends. The hours are 8 am - 4:30 pm, with some flexibility. An HHA at Our Hospice Will: Provide routine patient care in the patient's home or long term care facility per the Home Health Aide assignment in an exemplary manner, providing comfort, care, and compassion to patients and families. Assist the patient with daily hygiene activities (including, but not limited to: helping patients to shower, change clothing, shave, and toilet). Observe patients and reports observations and concerns to the primary care nurse and/or Patient Care Coordinator, Nurse Facilitator or Manager of Inpatient and Bartholomew.
