About the Role
We're looking for a builder - someone who wants to take a messy zero-to-one commercial motion and turn it into something that works, fast. As our first Head of Provider Growth, you'll own provider adoption end-to-end: testing channels, refining targeting, knocking on clinic doors with our first reps, and scaling the system once we know it works.
This role is perfect for someone who has built growth engines before (in startups, agencies, or their own companies), loves rapid experimentation, and wants to have massive impact in a short amount of time. What You'll Do - Impact in Your First 3-6 Months
Own the growth target. Build and execute a plan to reach meaningful PA volume across provider settings.
Run fast experiments. Test messages, channels, outreach methods, and install flows to identify what moves the needle fastest.
Get your hands dirty. Shadow field reps, visit clinics, talk to staff, learn the real workflows, and refine our pitch based on firsthand insight.
Target smarter. Partner with our Head of Strat/FinOps to map specialties, geos, and practice types with the highest PA density and frictionless install paths.
Create momentum. Build a simple but effective engine: outbound sequences, enrichment, list building, mini‑landing pages, CRM flows, funnel dashboards.
Produce early proof points. Deliver data that shows what works, what doesn't, and where we should double down.

Scale the playbook. Turn early wins into a repeatable, efficient GTM system.
Build the team. Hire the team you need to support and scale the motion you prove.
Optimize for leverage. Use automation, lightweight tools, and creative channel tactics to keep CAC low and velocity high.
Partner deeply with Product + Life Sciences. Share real‑world workflow insights that shape product decisions and help funder‑facing teams land partnerships.

You've built zero-to-one growth engines before - maybe at a startup, maybe at your own company, maybe as the first GTM hire somewhere.

SUMMARY OF THE POSITION:
Under the direction of the Area Supervisor, the Lead Primary Caregiver administers Early Head Start center operations, coordinates activities for infants and toddlers as guided by Head Start Performance Standards, adheres to procedures and policies, and follows California State Community Care Licensing regulations. AREAS OF RESPONSIBILITY:
* Provides in-home personal care and supportive services to Hospice patients. Personal care and supportive services include assisting a patient in and out of a shower, giving bed bath, sponge bath or shower, shampoo, shave, skin care, nail care, backrub, oral hygiene, passive range of motion, linen change - making an unoccupied bed and occupied bed; assisting with ambulation devices (cane, walker, wheelchair), transferring from bed to chair and from chair to bed, use assistive devices such as Hoyer lift; assisting a patient with toileting activities (use of commode, bedpan, urinal, toilet); emptying reservoirs such as Foley Catheter bags or colostomy bags; assisting with feeding patient; maintaining safe kitchen and bathroom; monitoring of vital signs and assist patient/family as specified by case manager or supervisor.
* Other supportive services include comfort measures and integrative therapies, such as aromatherapy and Comfort Touch. Comfort measures include providing companionship and conversation with patient and family members and activities such as reading to patient or sitting quietly with the patient, talking with the patient about patient's life or hobbies, and patient projects, such as organizing pictures in a book.
* Under the direct supervision of each patient's case manager, initiates the hospice aide care plan; provides feedback to case manager regarding changes in patient's physical, social, or psychological condition; and makes changes in care as directed.
* Assists in training individuals or families in the care of terminally ill patients.
* Maintains the dignity of the dying patient and supports the patient's and family's unique spiritual and cultural beliefs.
* Performs other tasks as requested by the case manager to assist the patient in obtaining needed health services or follow-up care.
* Accesses electronic device for care plan prior to patient visit.
* Completes appropriate records following all patient visits.
* Synchronizes electronic device data after the patient visit.
* Completes any other documentation requested by the agency.
* Synchronizes electronic devices for schedule after 2:30 pm on the day prior to visits and again the morning of the visits.
* Notifies Home Health Aide Department daily of intended visit route and visit times. Communicates promptly and appropriately with patients and families regarding scheduled visits.
* Communicates daily with Home Health Aide Department regarding scheduling changes or issues.
* Meets productivity standards established by the Home Health Aide Program Director.
* Maintains the ethical and confidentiality standards in the administration of Home Health Aide services.
* When in a facility (RCFE or SNF), utilizes customer service practice and department procedures for working in and with facilities.
* Maintains communication with a case manager, patient, family and facility staff regarding all aspects of patient care.
* Demonstrates awareness of and adherence to the safety, infection control and other policies of the facility.
* Attends and actively participates in the hospice interdisciplinary group (IDG) meetings. When attendance is not possible, provides appropriate input on assigned patients.
* Attends educational in-service trainings, department meetings and other agency meetings as requested by the manager.
* Maintains and updates the level of knowledge and skills required to provide quality hospice aide care.
* Adheres to all agency policies and procedures; including updates, as communicated in written, spoken and electronic forms.
* Submits time log when due.
* Promotes a culture of service when representing Pathways by at all times treating internal and external contacts with respect, remaining professional, communicating clearly, preventing conflict, following through on commitments and actively listening. As the Community Liaison for Institute on Aging's home care division, Corio, your primary objective is to generate qualified referrals through strategic business-to-business (B2B) sales initiatives. Working collaboratively with IOA teams (Corio, Companioa, Connect, and Marketing), you will ensure that every client referral aligns with IOA's exemplary service standards. Through fostering community partnerships, engaging in public relations activities, leveraging events, social media, and executing marketing initiatives, you will champion the positive image of IOA. Additionally, you will play a pivotal role in facilitating the intake to enrollment processes for prospective clients, their fiduciaries, and referral sources.
Essential Job Functions:
Develop and execute a Strategic Account Plan for B2B community partnerships, nurturing relationships, tracking outreach activities, and following IOA's consultative sales process.
Cultivate and sustain relationships with referral sources to enhance brand awareness and increase enrollments for IOA Home Care (Corio) and Services for those living with Dementia (Companioa) programs. Ensure ongoing business satisfaction through routine follow-ups and relay feedback to program leaders.
Maintain comprehensive knowledge of Corio and Companioa service offerings, consistently conveying messaging through social media and community presentations.
Handle incoming inquiries, assess prospects' needs, provide information, and facilitate necessary follow-ups with team members.
Conduct assessments as required, building rapport with prospects and their families, addressing concerns, and converting qualified leads to enrolled clients.
Champion delivering the ideal client journey at every opportunity, from meeting with clients to working with and inspiring internal staff.
Enter prospect and client information accurately and promptly into the sales automation system (HubSpot) and the electronic health record (WellSky) as appropriate, including follow-up notes and referral sources.
Stay informed about Corio and Companioa programs' strategic positioning in the market, identifying opportunities for reputation enhancement and referral base expansion.
Collaborate with program leaders to achieve census goals, resolving enrollment process barriers promptly to ensure timely client enrollment.
Assist in generating monthly data on key outreach and enrollment metrics. Review and update qualified leads reports in the HubSpot and WellSky, as appropriate.
Develop and maintain an effective network of community resources for client referrals, keeping Corio and Companioa team members informed.
Provide after-hours support to referral partners, prospective clients, and active clients as necessary.
Participate in weekly pipeline meetings to review prospects, devise enrollment strategies, and determine follow-up actions for B2B relationship management.
Participate in weekly or biweekly sales coaching meetings
Lead weekly business development meetings with Corio and Companioa teams, providing oversight to support outreach efforts.
Attend weekly Marketing and Sales Key Performance Indicator (KPI) meetings with the CMO and VP of home care and ensure departmental goals are met.
Actively engage in additional team meetings as required.
Perform other duties as assigned.
Qualifications and Skills:
Bachelor's degree preferred or equivalent experience in communications, business, gerontology, or a related field.
Previous experience in healthcare management, inside sales and/or outside sales or marketing, preferably in Homecare, Assisted Living, or a related field desired.
Demonstrated knowledge of the senior care industry.
Ability to work independently with strong organizational skills, assertiveness, and the capacity to prioritize tasks, manage schedules, and meet deadlines.
Effective relationship-building skills with internal and external clients and constituents.
Proficiency in applying consultative (needs-based selling) sales approaches.
Strong written and oral communication skills, including presentation abilities.
Computer proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.
COMPENSATION
Range: $78,580.02 - 85,000/Annual
