General Purpose The primary purpose of your job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors. Use PCC to ensure correct resident information when assisting with ALDS, meals, etc. Report all changes in the residents' condition to the Health and Wellness Director as soon as practical. Record all entries on notes, SPAs, and incident reports , etc., in an informative, descriptive manner. Report on all accidents and incidents you observe on the shift that they occur to the Executive Director and Health and Wellness Director . Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Executive Director . Report any known or suspected unauthorized attempt to access communit y's information system. Perform only those care tasks that you have been trained to do. Ensure that the resident's room is ready for the resident (i.e., bed ma de ). Greet residents and escort them to their room. Introduce residents to his/her roommate, if any, and other residents and personnel as appropriate. Make residents comfortable. Inventory and mark the residents' personal possessions as instructed. Store resident's clothing. Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. Transport residents to new rooms or to the receiving area. Assist with loading/unloading residents' to/from vehicles as necessary. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. Report all complaints and grievances made by residents. Participate in and receive the nursing report as instructed. Follow established policies concerning exposure to blood/body fluids. Make beds as instructed. Put extra covers on beds as requested. Ensure that residents who are unable to call for help are checked frequently. Answer resident calls promptly. Check residents routinely to ensure that their personal care needs are being met. Assist residents with identifying food arrangements (i.e., informing residents with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). Keep residents' water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, etc.). Receive the nursing report upon reporting for duty. Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. Follow work assignments, and/or work schedules in completing and performing your assigned tasks. Cooperate with inter-departmental personnel, as well as other facility personnel, to ensure that services can be adequately maintained to meet the needs of the residents. Notify the facility when you will be late or absent from work. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents. Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled). Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.). Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Perform restorative and rehabilitative procedures as instructed. Assist in preparing the resident for a physical examination. Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). Report injuries of an unknown source, including skin tears. Observe disoriented and comatose residents. Record and report data as instructed Perform special treatments as instructed. Immediately notify the Nurse of any resident leaving/missing from the facility. Use only the equipment you have been trained to use. Report defective equipment to the Nurse Supervisor/Charge Nurse Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. Report all allegations of resident abuse and/or misappropriation of resident property. Honor the resident's refusal of treatment request. Report such requests to your supervisor. Give back rubs as instructed Assist residents with dressing\undressing as necessary Assist residents with hair care functions (i.e., combing, brushing, shampooing, etc.) Assist residents with nail care (i.e., clipping, trimming, and cleaning the finger/toenails) Shave male residents Keep hair on female residents clean (i.e., facial hair, under arms, on legs, etc.) as instructed. Keep residents dry (i.e., changing gown, clothing, linen, etc., when it becomes wet or soiled) Make beds (occupied and unoccupied) Put extra covers on beds as requested Position bedrest residents in correct residents in correct and comfortable positions. Assist residents with bowel and bladder functions (i.e., take to bathroom, offer bed pan, urinal, portable commode, etc.) Maintain intake and output records as instructed. Keep incontinent residents clean and dry. Check and report bowl movements and character of stool as instructed Perform restorative and rehabilitative procedures as instructed. Utilize protective and adaptive equipment as instructed. Provide eye and ear care (i.e., hearing aide installation, cleaning eyeglasses/hearing aids, etc.) as instructed. Assist in preparing the resident for physical examination. Weigh and measure residents as instructed. Measure and record temperatures. Pulse, and respirations (TPRs), as instructed. A caregiver is a primary provider of resident care, assisting with all activities of daily living as needed according to an individualized care plan. A caregiver promotes the physical, personal, and emotional well-being of each resident and strives to maintain the residents at the maximum level of self-care and independence. The goal of quality resident care is to enable each resident, where possible, to return to the highest level of functioning, to attain a new stage of independence, or to at least prevent regression. An active approach to resident care will be taken and not just passive maintenance. Essential Responsibility Follow individualized plan of care (ISP) for each assigned resident. Observe residents for change in condition and report any changes immediately to nursing supervisor. Document daily care and red flags throughout the shift, especially rounds at night as they occur. Check and record vital signs and weights, as assigned. Collect urine, stool, and sputum specimens as assigned. Assist with bowel and bladder needs, as per resident plan of care. Provide or assist residents with activities of daily activities as needed, including bathing/showering according to schedule, oral care, toileting, dressing etc. Perform restorative nursing techniques including proper positioning, ADL training, and Passive Range of Motion. Assist in providing appropriate information and behavioral prompts, cues, redirection, and affirming support to residents in need. Encourage residents to participate in scheduled activities; assist residents in getting to activities. Coordinate and assist with activities as directed. Assist residents with transfers and ambulation as directed, using proper body mechanics, back supports, and mechanical devices as required. Provide transportation for residents within the facility, and at times, accompany appointments outside facility. Respond to all calls on the floor whether they be urgent or not. All calls must be responded to. Provide back-up support to other departments, as needed. Work as a team with nursing in support of resident care. Assist in problem solving in communication or meetings with nurse and or Health Services Director. Empty trash in resident's rooms and other resident areas as needed. Keep residents' rooms, dining area, utility room, and other areas of the facility clean and orderly. Keep the environment safe for residents (hallways clear of equipment and floors free of debris and linen) Change bed linens according to facility policy. Provide laundry services, as necessary. Requisition and charge resident supplies as needed. Always maintain a restraint free environment. Support fall prevention programs for all residents in need. This includes proactive intervention use, monitoring, and safety in the rooms and rounds. Help welcome and transfer residents according to facility procedure. Assist in post-mortem care as instructed. Obtain and demonstrate knowledge of the philosophy of Commitment, Respect, Compassion, Dedication, Teamwork and Quality. Perform other duties as assigned and as outlined in policies and procedures, disaster plan, and fire plan. Attend in-service classes and staff meetings. Ability to work with the elderly in a courteous and friendly manner, demonstrating patience and compassion towards the elderly. Ability to always perform duties with consideration for residents' rights at all times and demonstrate integrity and confidentiality in the care of residents and in handling their health information. Technical Skills: Knowledge of medical terminology, medical records management, medical supplies, and equipment. Ability to perform basic adult CPR and basic adult first aid procedures. Physical/Environmental Demands Physical/Sensory: Daily, throughout the workday, must be capable of sitting, standing, walking, climbing stairs, reaching, turning, bending, stooping, crouching, and kneeling, reaching over the head, grasping, with both hands, and fine manipulation. Must physically be able to lift to 25 pounds unassisted. Must physically be able to carry, transfer, push and pull, and reposition residents with assistance or using appropriate equipment. Must have normal eyesight or use corrective lenses. Must have three-dimensional vision and ability to judge distances and spatial relationships to see objects where and as they actually are. Must be able to recognize colors. Must be able to hear and distinguish between normal tones and be able to perceive the nature of sounds. Must be able to exchange ideas by means of the spoken word as well as engage in activities to convey detailed or spoken instructions to other workers accurately, loudly, and/or quickly. Must have normal sense of touch and smell. Work Environment: Works in a well-lit, well-ventilated building. PHYSICAL THERAPIST ASSISTANT A Physical Therapist Assistant (PTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager/Therapy Supervisor and supervising Physical Therapist. Qualifications QUALIFICATIONS 1. A person who is licensed, registered or certified as a physical therapist assistant, if applicable, by the state in which practicing, unless licensure does not apply and meets one of the following requirements: a. Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association. OR b. If educated outside the United States or trained in the United States military, graduated from an education program determined to be substantially equivalent to physical therapist assistant entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or identified at 8 CFR 212.15(e). AND Passed a national examination for physical therapist assistants. OR c. On or before December 31, 2009, meets one of the following: i. Is licensed, or otherwise regulated in the state in which practicing. ii. In states where licensure or other regulations do not apply, graduated before December 31, 2009, from a 2-year college-level program approved by the American Physical Therapy Association and after January 1, 2010, and has passed any state required examinations for licensure. OR iii. Before January 1, 2008, where licensure or other regulation does not apply, graduated from a 2-year college-level program approved by the American Physical Therapy Association OR iv. On or before December 31, 1977, was licensed or qualified as a physical therapist assistant and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service. 2.
