About the Role
We're looking for a builder - someone who wants to take a messy zero-to-one commercial motion and turn it into something that works, fast. As our first Head of Provider Growth, you'll own provider adoption end-to-end: testing channels, refining targeting, knocking on clinic doors with our first reps, and scaling the system once we know it works.
This role is perfect for someone who has built growth engines before (in startups, agencies, or their own companies), loves rapid experimentation, and wants to have massive impact in a short amount of time. If you're energized by figuring things out, learning new markets quickly, and driving toward a real number, you'll thrive here.
What You'll Do - Impact in Your First 3-6 Months
Own the growth target. Build and execute a plan to reach meaningful PA volume across provider settings.
Run fast experiments. Test messages, channels, outreach methods, and install flows to identify what moves the needle fastest.
Get your hands dirty. Shadow field reps, visit clinics, talk to staff, learn the real workflows, and refine our pitch based on firsthand insight.
Target smarter. Partner with our Head of Strat/FinOps to map specialties, geos, and practice types with the highest PA density and frictionless install paths.
Create momentum. Build a simple but effective engine: outbound sequences, enrichment, list building, mini‑landing pages, CRM flows, funnel dashboards.
Produce early proof points. Deliver data that shows what works, what doesn't, and where we should double down.

Scale the playbook. Turn early wins into a repeatable, efficient GTM system.
Build the team. Hire the team you need to support and scale the motion you prove.
Optimize for leverage. Use automation, lightweight tools, and creative channel tactics to keep CAC low and velocity high.
Partner deeply with Product + Life Sciences. Share real‑world workflow insights that shape product decisions and help funder‑facing teams land partnerships.
Who You Are
You've built zero-to-one growth engines before - maybe at a startup, maybe at your own company, maybe as the first GTM hire somewhere.
You're comfortable doing the hands‑on work: outreach, targeting, data pulls, experiments, dashboards, even jumping into a clinic visit.
You love learning new markets fast and figuring out “where the volume is.”
You'd rather launch something messy tomorrow than plan something perfect for next month.
You bias toward velocity, iteration, and ownership.
You're energized by the idea of owning a number and building the system that achieves it.

5+ years in growth, GTM, marketing, market development, sales ops, or similar roles with real execution responsibility.

Strong analytical instincts: segmentation, funnel analysis, basic sheets, experiment testing frameworks.

Position Overview
The Home Health Aide (HHA) provides essential personal care and support services to clients in their homes under the supervision of a Registered Nurse or Nurse Supervisor. The HHA's tasks are non-invasive, focused on daily living assistance, and must align with the nursing care plan. The role does not include the administration of medications or the performance of clinical procedures requiring licensure. HHAs may include Certified Nursing Assistants (CNAs), Licensed Practical Nurses (LVNs), or RNs providing aide-level care under this scope.
Essential Duties And Responsibilities
DUTIES
Provides personal care and related services in the home, under the direction, instruction and supervision of the staff nurse and the Nurse Supervisor
Tasks to be performed by a HHA must be assigned by, and performed under, the supervision of an RN who will be responsible for the patient care provided by the HHA.
Home Health Aide will not administer medications, take physician's orders, receive, or reduce any intravenous procedures, or any other sterile or invasive procedures, other than rectal temperatures or enemas, or perform procedures requiring the training, knowledge, and skill of a licensed nurse.
Home Health Aide services may also be provided by a CNA (Certified Nursing Assistant) subject to the same supervision described below. If necessary, they may also be provided by an LVN or RN.
RESPONSIBILITES
Follows the plan of care to provide, safe, competent care to the client
Helps the patient to maintain good personal hygiene and assists in maintaining a healthful, safe environmen
Plans, and prepares, nutritious meals, grocery shops, when instructed to do so, by the nurse
Assists the patient with ambulation, as ordered, by the physician, and approved, and supervised, by the nurse
Assists the therapy personnel, as needed, with rehabilitative processes
Encourages the patient to become as independent as possible, according to the nursing care plan
Attempts to promote patient's mental alertness, through involvement in activities of interest
Gives simple emotional, and psychological, support to the patient, and other members of the household, and establishes a relationship with patient and family, which transmit trust and confidentiality
Reports any change in the patient's mental, or physical, condition, or in the home situation, to the staff nurse, or to the Aide supervisor
Performs routine housekeeping tasks, as related to a safe and comfortable environment for the patient, as instructed by the professional nurse
Prepares a visit report, promptly, and incorporates same in the clinical record, weekly
Confirms, on a weekly basis, the scheduling of visits so that other necessary visits, by staff members, can be coordinated
Works with personnel of other community agencies involved in the patient's care, as directed by the nurse
Attends in-service, as required by regulation or Agency policy
Has access to all client medical records which may be discussed with the Registered Nurse and the Nurse Supervisor
Job Conditions
The ability to drive and the ability to access clients' homes which may not be routinely wheelchair accessible are required.
Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 50 pounds.
Must be able to communicate clearly, both verbally and in writing in English.
Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.
Qualifications
High school diploma or equivalent
Must have completed a minimum of 75 hours of training, which includes an introduction to personal care services
If a Nursing Assistant, certified by the state, (CNA), the individual will meet the qualifications for a Home Health Aide, as his or her hours of training will exceed the minimum required.
Must understand, and respect, clients, including ethics and confidentiality of care.
Must have current CPR certification. Online certification is not accepted.
Must have criminal background check
Must be free from health problems that may be injurious to patient, self, and co-workers and must present appropriate evidence to substantiate this
Must comprehend the basics of personal care, housekeeping, and meal preparation
The ability to communicate well, both, verbally, and in writing, in English, is required.
As part of our final selection process, candidates are required to provide two professional references and one personal reference. Please note that an offer of employment is contingent upon the successful validation of these references by our hiring team.
Environment
Contact with clients under a wide variety of circumstances.
Care is provided in client's living environment with varying situations, including private homes, independent or assisted living or in an institution such as a hospital or long-term care facility.
May be required to respond to emergency situations.
Position includes tasks that have the potential for exposure to blood/body fluids/tissues and other potentially infectious material such as body fluids/tissues.

Physical Requirements
Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.
Must be able to lift up to 50 lbs.
Corrected visual and hearing acuity required.

Must pass medical clearance and background check
Must be able to communicate clearly and effectively in verbal and written English
Work Hours
Work hours will vary depending on the client's needs. Work hours may include day, evening, weekend, or holiday shifts, based on client needs and the availability of the employee.

JOB SUMMARY:
The Home Health Aide provides professional nursing services under the supervision of a Registered Nurse in compliance with the state's Nurse Practice Act, governmental laws and regulations, and Elite Health Care Systems policies and procedures. The Home Health Aide is a professional member of the interdisciplinary group that performs various services for a patient as necessary to meet the patients personal needs and to promote comfort.
DUTIES AND RESPONSIBILITIES:
Under the direction & ongoing supervision of the RN, provides essential personal care assistance to the patient: Bath, skin, and hair.
Bed transfers as well as ambulation activities as ordered under the care plan
Preferring such incidental household services as are essential to the patients well-being at home
Preparing meals including special diet
Take a patients temperature, pulse, respiration, and blood pressure if assigned by a registered nurse
Assists with shopping if the patient has no other resources available
Provides emotional support and compassionate care to the patient and family unit
Demonstrates listening skills
Offers encouragement when appropriate
Maintains required competency and skills
Documented initial and ongoing every 14 days supervision and evaluation by RN on each case assignment
Participates in an annual performance review
Report any changes in the patients condition immediately by calling the Hospice office and notifying the RN Case Manager.
Practice Standard Precautions according to OSHA regulations and policy
Attends at least twelve (12) hours of in-service education programs per year
Maintain current certification as required by the State
Maintains current knowledge and practice of infectious disease protocols
Performs personal care needs after given a patient's assignment and attending to his/her requests promptly.
Meeting safety needs of the patient and using equipment safely and properly (foot stools, side rails, Oxygen, etc.)
Providing respite for patient's family/caregiver when on-site, as appropriate.
Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
QUALIFICATIONS:
Minimum 1-year experience as a Home Health Aide or Hospice Aide.
Must have Certification as CHHA.
Understands the philosophy of Hospice concepts. Experience with the terminally ill patient.
Ability to handle multiple priorities, documentation requirements and deadlines.
Must have reliable automobile, valid drivers license, and the minimum state-required liability auto insurance.
Able to travel to multiple locations in the course of the working day/week.
Ability to assess and respond to the needs of patients, families, and caregivers and to function effectively in a nursing environment.
Has excellent observation, verbal and written communication skills.
Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
Proficient computer skills required using specialized software, and equipment used in the delivery of patient care.
Committed to improvement, clinical excellence, high-degree of patient and staff satisfaction.
Demonstrates excellence in customer service and time management.
Possession of current certification to practice as a Home Health Aide in the State of California.
