Description: This position provides custodial and personal care services to clients that contract with Assisting Hands for services under the direction of the Staffing Coordinator in compliance with the Plan of Care. Reports to: Staffing Coordinator Primary Duties: Maintaining a clean, safe, and healthy environment within clients' residence. Duties may include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing client's dishes or laundry, and other tasks to maintain cleanliness and safety for the client. The performance of all personal care activities contained in a written Plan of Care which may include assisting the client with personal hygiene, ambulation and exercise, eating, dressing, shaving, physical transfer, and other duties as assigned. Providing nutritional support as needed. General Purpose The primary purpose of your job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors. Use PCC to ensure correct resident information when assisting with ALDS, meals, etc. Report all changes in the residents' condition to the Health and Wellness Director as soon as practical. Record all entries on notes, SPAs, and incident reports , etc., in an informative, descriptive manner. Report on all accidents and incidents you observe on the shift that they occur to the Executive Director and Health and Wellness Director . Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Executive Director . Report any known or suspected unauthorized attempt to access communit y's information system. Perform only those care tasks that you have been trained to do. Ensure that the resident's room is ready for the resident (i.e., bed ma de ). Greet residents and escort them to their room. Introduce residents to his/her roommate, if any, and other residents and personnel as appropriate. Make residents comfortable. Inventory and mark the residents' personal possessions as instructed. Store resident's clothing. Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. Transport residents to new rooms or to the receiving area. Assist with loading/unloading residents' to/from vehicles as necessary. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents. Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). A caregiver is a primary provider of resident care, assisting with all activities of daily living as needed according to an individualized care plan. A caregiver promotes the physical, personal, and emotional well-being of each resident and strives to maintain the residents at the maximum level of self-care and independence. The goal of quality resident care is to enable each resident, where possible, to return to the highest level of functioning, to attain a new stage of independence, or to at least prevent regression. An active approach to resident care will be taken and not just passive maintenance. Essential Responsibility Follow individualized plan of care (ISP) for each assigned resident. Observe residents for change in condition and report any changes immediately to nursing supervisor. Document daily care and red flags throughout the shift, especially rounds at night as they occur. Check and record vital signs and weights, as assigned. Collect urine, stool, and sputum specimens as assigned. Assist with bowel and bladder needs, as per resident plan of care. Provide or assist residents with activities of daily activities as needed, including bathing/showering according to schedule, oral care, toileting, dressing etc. Perform restorative nursing techniques including proper positioning, ADL training, and Passive Range of Motion.
