b. Bathing, Grooming, & Hygiene- Provides client hands-on and/or stand-by assistance with bathing, showering and other grooming activities (may include bed baths/sponge baths when client is unable to use the shower/bathtub).
c. Meal Preparation/Nutritional Support- Follows nutritional and meal planning guidelines to assist with preparation and serving of breakfast, lunch, dinner and/or snacks to ensure proper nutritional intake (including dietary supplements).
d. Dressing- Provides hands-on and/or stand-by assistance with changes of client's clothing as needed.
e. Toileting- May include assisting on/off toilet or commode; assisting with use of urinal or bedpan; emptying catheter or colostomy bag; changing diapers/briefs; and ensuring cleanliness of client as needed.
2. Oral medication reminders (caregivers are not permitted to administer medication)
3. Light housekeeping duties in order to maintain a safe and clean environment
a. Launders client's clothing, bed sheets, linens and towels as requested (includes washing, drying, folding and putting clean items away).
b. Maintains cleanliness of client's bathroom by cleaning and disinfecting the toilet, shower, bathtub, sink, counters and floor.
c. Maintains cleanliness of kitchen by properly disposing of expired food, washing dishes, and cleaning the sink, counters, stove top, floor, and other surfaces (NOT responsible for oven cleaning).
d. Maintains tidiness of client's bedroom and living areas by tidying up, changing bed sheets, vacuuming/sweeping/mopping floors, dusting surfaces, and emptying trash receptacles.
4. Transportation/escort to appointments
a. Drives client (and spouse if requested) to scheduled appointments in the local community (use of caregiver's car is preferable).
b. Escorts client into appointment location and assists with ambulation/transfers as needed.
c. Remains in the waiting room or within a reasonable distance if client prefers privacy.
5. Supervision and companionship
a. Engages client in appropriate conversations (caregiver is to avoid personal disclosures about self, and discussions about client's family).
b. Takes client on walks near the client's home.
c. Entertains client by reading newspapers or books, playing memory games, completing puzzles, etc.
6. Assistance with Range of Motion exercises as requested (Initial demonstration by a family member, physical therapist, or other licensed healthcare professional is recommended)
7. Compliance with all state laws and agency policies related to the timely mandated reporting of suspected child, dependent adult and elder abuse and domestic violence
8. Compliance with agency policies related to all areas of privacy and confidentiality, including HIPAA laws
9. Observance of safety guidelines as outlined in the Employee Safety Manual
10. Timely reporting of any unusual occurrence in client's home or change in client's condition to supervisor (agency must be called immediately upon such knowledge)
11. Prompt arrival to and completion of all shifts as assigned (should a client request a schedule change, supervisor must be notified as soon as possible and prior to implementation of the change)

Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator. Report any known or suspected unauthorized attempt to access facility's information system. Perform only those care procedures that you have been trained to do. Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). Greet residents and escort them to their room. Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. Make resident comfortable. Inventory and mark the resident's personal possessions as instructed. Store resident's clothing. Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged.
