The Family Engagement Coordinator (FEC) ensures that an individual's wishes regarding organ and tissue donation are understood and honored whenever possible. When an individual's choice is not known or documented, the FEC supports the family in understanding their right to make a decision on their loved one's behalf. FECs provide balanced, clear, and respectful information about donation, allowing families to make decisions that reflect the individual's intent and, when applicable, the family's values. The role requires high emotional intelligence, composure in difficult moments, and a working understanding of requirements set by CMS, OPTN, the New York State Anatomical Gift Act, the FDA, and related standards.
The Family Engagement Coordinator (FEC) serves in a full-time, on-site operational role. The designated company office for this position is the donor hospitals, where the FEC conducts a significant portion of their responsibilities in direct support of family engagement and time-critical donation activity. When operational needs require, the FEC may be assigned to the Corporate Office or, when specifically authorized, may perform approved responsibilities from their home. All work locations must remain fully aligned with LiveOnNY's policies.
Responsibilities
Advocating for Individual and Family Choice
- Uphold and support the individual's documented or expressed donation decision.
- When the individual's choice is not known, ensure the family understands their right to choose on their loved one's behalf.
- Provide clear and accurate information about organ and tissue donation.

Supporting Families with High Emotional Intelligence

Donation Facilitation & Consent Discussions
- Collaborate with clinical teams and hospital staff to determine appropriate timing for donation conversations.

- Conduct the Donor Risk Assessment Interview (DRAI) with the most appropriate individual(s).
- Ensure consent and authorization processes meet LiveOnNY, hospital, and regulatory expectations.
Collaboration with Clinical Services & Hospital Staff
- Participate in care huddles, pre-authorization reviews, and post-authorization discussions.
- Communicate family dynamics, decision status, and relevant updates to clinical counterparts.
- Serve as a first responder on referrals as needed and share essential chart information with Clinical Services.
Documentation & Regulatory Alignment
- Complete accurate, timely documentation of family discussions, authorization outcomes, and required forms.
- Ensure all processes align with expectations set by CMS, OPTN, the New York State Anatomical Gift Act, the FDA, and other relevant standards.
- Maintain confidentiality and adhere to ethical practices at all times.
Education, Training & Organizational Learning
- Assist in the orientation and training of new Family Engagement Coordinators, Donation Coordinators, and Hospital Services Specialists.
- Provide education to hospital staff regarding donation discussions and the role of the FEC.
- Maintain current knowledge of donation practices, emotional-support techniques, and relevant regulations.

Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support.
Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. The Physician will be part of a core interdisciplinary team working in a unique environment delivering the care to patients with significant chronic illnesses. The Physician will provide general primary care including conducting histories and physicals, diagnosing and treating acute and chronic illnesses, and evaluating the need for consult services. The interdisciplinary team will work under supervision of a Site Medical Director.
Responsibilities include:
Diagnose and treat acute and chronic illnesses. Evaluate the need for consult services and submit the prioritized consult when indicated.
Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable.
Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits.
Implement plans for patient care utilizing protocols approved by the medical leadership and/or treatment plans reflecting the current standard of care.
Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate.
Collaborate closely with CHS Physician Assistants, including providing clinical guidance, cosigning notes, and providing other supervision based on clinical circumstance and PA requirements.
Review all specialty consults and hospital returns to ensure that the standard of care is met and recommendations of the consultant are implemented.
Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed.
Generate special needs referrals and documentation as needed (for patients with (disabilities, dietary restrictions, heat sensitivity, or other relevant flags).
Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care.
Ensure that all progress notes and orders are signed before the end of the shift.
Respond to emergencies in a timely and professional manner.

In this role, you will:
Identify and engage caregivers of adults who are 65 and older through admission/rounds flags and referrals; obtain consent and explain program scope.
Conduct brief structured screens using evidence-based tools (e.g., PHQ‑2/9, GAD‑2/7, brief caregiver‑burden check, health‑literacy and SDOH prompts).
Facilitate a concise “What Matters” conversation to elicit the caregiver's priority needs and preferences; co‑create an action plan focused on1-3 priority areas.
Provide tailored resource packets (education, benefits, respite, mental health, community‑based supports); use teach‑back to ensure understanding and feasibility.
Coordinate with the care team (e.g., primary team, case management, nursing, behavioral health) to align plans and communicate a concise summary note.
Track outputs and outcomes (encounters, screening completion, referrals provided, closed‑loop hand‑offs, caregiver‑reported stress/depressive symptoms when available).
We require:
MSW from an accredited School of Social Work; NYS licensure or permit as applicable.
Minimum 1 year in medical, geriatric, primary care, or community health social work experience with brief screening, goal‑setting, and resource navigation preferred.
Motivational interviewing or equivalent brief counseling; familiarity with evidence-based screening tools (eg, PHQ/GAD screening); knowledge of accessible and relevant caregiver resources (respite, benefits, mental health); EMR proficiency; strong communication and teamwork.

Surgical Recovery Coordinator Responsibilities include:
Coordinate organ recovery and preservation procedures with Transplant Coordinators, Donor Hospital OR staff, and Organ Recovery Team(s).
Participate in organ donor recoveries that require traveling to donor hospitals by aircraft and arrive at the donor hospital on time for the organ recovery.
Provide appropriate equipment, solutions, and pre-operative supplies for recovery and preservation of solid organs in local donor hospitals and during “fly outs.”
Perform surgical/OR techniques during organ recovery as they relate to organ preservation.
Maintain organ preservation lab and restocks equipment, supplies, solutions, and other inventory as needed.
Perform machine pulsatile organ preservation techniques as directed.
Qualifications
Surgical Recovery Coordinator Qualifications:
MD, RN, Physicans Assistant, Nurse Practioner, Surgical Technician, EMT or equivalent.
1-2 years related experience and/or training preferred.
CTP (Certified Transplant Preservationist) preferred.
Good knowledge of human anatomy/function and basic medical terminology is necessary. Knowledge of surgical aseptic technique and demonstrated ability to work in a sterile operating room setting required.
Must be able to travel within our Greater New York service area (NYC and surrounding counties).
