Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
Participate in planning for and the execution of patient discharge experience.
Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.

Complete case management addendums and all required documentation.

Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
Make appropriate/timely referrals, including documentation to post discharge providers/physician
Ensure accuracy of discharge and payor-related information in the patient record
Participate in utilization review process: data collection, trend review, and resolution actions.
Participate in case management on-call schedule as needed.
Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM⢠certification upon entry into this position OR within two years of entry into the position.
CCM or ACM⢠certification required OR must be obtained within two years of being placed in the Case Manager II position.
Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

Summary:

Essential Work Functions:
Prepare and position patients for cardiac catheterization, angioplasty, stent placement, electrophysiology studies, and other interventional procedures

Operate and maintain imaging equipment, contrast injectors, and hemodynamic monitoring systems, ensuring proper functionality
Scrub in and assist with sterile field setup, ensuring adherence to infection control and procedural protocols
Perform sheath removal and hemostasis using manual compression or closure devices as directed
Maintain accurate documentation, including patient records, imaging data, and equipment logs

Summary:
The Maintenance and Mechatronic Technician will be responsible for maintaining, troubleshooting, and repairing automated systems, robotic equipment, and mechanical machinery within the facility. The technician will work closely with production teams to ensure minimal downtime and optimize the performance of equipment. This role requires an understanding of mechanical, electrical, and electronic systems, as well as the ability to read technical drawings, schematics, and manuals.
Responsibilities:
Perform routine and preventive maintenance on a variety of industrial equipment, including mechanical, electrical, hydraulic, and pneumatic systems.
Diagnose and repair equipment malfunctions to minimize downtime and ensure production efficiency.
Conduct regular inspections and maintenance of PLC systems to ensure they are functioning properly.
Modify and update PLC programs as required to improve machine performance or accommodate production changes.
Document any changes made to PLC programs and ensure that updated versions are properly stored and backed up.
Assist in the installation and commissioning of new equipment, including the setup of PLC systems.
Monitor the performance of production machinery and systems, using diagnostic tools to detect issues before they lead to equipment failure.
Maintain accurate records of all maintenance activities, including equipment repairs, parts used, and time spent on tasks.
Generate maintenance reports and communicate updates to the Production Manager.
Adhere to all safety procedures and guidelines when performing maintenance tasks to prevent accidents and injuries.
Participate and contribute in a team environment by working with all levels of team members.
Knowledge, Skills, and Abilities:
Must have 2 or more years of preventative maintenance experience.
Must have working technical knowledge including hydraulic systems, pneumatic systems, electrical repair, and PLC
Strong troubleshooting and diagnostic skills for both mechanical and electrical systems.
Proficiency in PLC programming and troubleshooting.
Ability to read and interpret technical manuals, blueprints, and electrical schematics.
Strong attention to detail and the ability to work independently.
Strong communication and collaboration abilities.
Demonstrated ability to utilize Microsoft Office software; ability to learn and maintain CMMS software.
Ability to physically complete required tasks including standing for extended periods of time, stoop, bend, climb ladders/stairs, and occasionally lift and/or move up to 75lbs.
Ability to effectively utilize a computerized maintenance management system (CMMS)

Base Pay and Benefits:
An hourly commensurate with experience will be provided
