Compensation & Benefits

General description
Under general supervision, determines families' eligibility for subsidized child care programs and other Early Care and Education programs offered through the Santa Barbara County Education Office. Enrolls and registers eligible children and families and provides a wide range of services and support to program participants.
Specific Duties and Responsibilities
Determines level of need, family income, and eligibility for subsidies; enrolls children and families into the program.
Assists families in completing enrollment applications; may complete applications or prepare other required documentation on behalf of applicants with limited literacy; reviews documentation for accuracy and contacts families to correct discrepancies.
Applies family fees according to income level, family size and other needs; all according to fee schedules established by the California departments of Social Services or Education.
Provides comprehensive support to families by making referrals to other subsidies, services, and support for which they may be eligible; serves as a family advocate.
Advises families on program requirements, procedures and obligations for subsidized child care on a sustained basis.
Partners with other agencies to maintain up-to-date client file data such as employment, medical, and other information.
Serves as point-of-contact for parents to arrange and track their participation in child care activities and verify participation that complies with program obligations.
Assists with periodic program evaluations by participating in internal audits of case load files, contracted providers, and eligibility compliance.
Prepares communications with families including those for terminating services.
Represents SBCEO during community outreach events to promote services for families.
Performs other duties as assigned that support the overall objective of the position.
Requirements
Education: Possession of a high school diploma is required. Completion of 12 semester units of college coursework in business, public, or non-profit administration; social sciences, human services, or related field is preferred.
Experience: Two years of clerical or direct service experience in human or social services.

Job Summary:
The CaseManager role involves managing member intake for specialized programs and grants, ensuring compliance with grant reporting requirements, and providing casemanagement for members. Essential Duties & Responsibilities:
Attends site and/or in-service meetings for the purpose of conveying and/or gathering information required to perform functions.
Establish and maintain case files of assigned mentee caseloads.
Coordinates member participant outreach by making phone calls and scheduling meetings.
Maintain a variety of manual and electronic documents, files, and records for the purpose of providing up-to-date information and historical reference in accordance with established administrative guidelines and legal requirements. This may include participant information.
Assists with assessing members' needs, situations, personal strengths, and support networks to determine their goals.
Facilitates social-emotional development curriculum to include SMART Moves, Substance Use Prevention, and other assigned curricula.
Coordinates with appropriate organizations and services, including, but not limited to, social services, physical and mental health services, housing support, employment education, and substance use disorder treatment.
Researches and refers participants to community resources.

You'll Be Qualified If You Have
Bachelor's degree from an accredited institution of higher learning is required; Master's degree with emphasis in special education highly preferred

Possession of either BCLAD or CLAD certification highly desirable
3 years of effective casemanagement skills required

Provide in-depth, wrap-around casemanagement services to a caseload of youth in placement and in the community.

Job Summary: The CaseManager coordinates and oversees services for adults with developmental disabilities in our behavior management day program. In this role, you will be the primary advocate and planner for a caseload of program participants. You will develop individualized service plans, coordinate with families and external providers, and ensure each participants needs are met in accordance with their goals and behavioral support plans. The CaseManager is essential in ensuring quality of care, compliance with regulations, and smooth communication among all parties. This position is ideal for a detail-oriented professional who is passionate about empowering individuals with disabilities and has strong organizational and advocacy skills.
Key Responsibilities:
Develop Individual Plans: Create and maintain individualized service plans (ISPs) or program plans for each participant, in collaboration with the participant, their family, and the team. Ensure plans align with each persons IPP goals and behavioral support needs.
Service Coordination: Coordinate all services and supports for participants. Act as a liaison with families, residential caregivers, Regional Center coordinators, and other service providers to arrange needed supports (therapies, medical appointments, transportation, etc.).
Monitor Progress: Track participant progress towards goals. Collect input from direct support staff and behavior specialists on participant achievements and challenges. Regularly review and update plans to reflect progress or changing needs.
Documentation & Compliance: Maintain thorough case files and documentation for each participant, including progress notes, quarterly reports, and any incident reports. Ensure all documentation meets the standards and regulations for adult day programs (e.g., regional center and state requirements).
Team Collaboration: Work closely with counselors/direct support staff and the ABA Specialist to ensure that daily programming and behavior strategies are implemented according to each participants plan. Provide guidance and training to staff on individual participant needs and plans.
Advocacy: Advocate for participants needs and rights. This includes facilitating participant meetings (such as annual IPP meetings or interdisciplinary team meetings), voicing any concerns about participant welfare, and helping problem-solve any barriers to success.
Quality Assurance: Ensure the health, safety, and well-being of participants at all times. Participate in program evaluations and help implement improvements. Stay up-to-date with best practices and regulations in the field of developmental disability services.
Other Duties: Perform other casemanagement and program support tasks as assigned by the Program Director to help the program run smoothly.
Qualifications & Requirements:
Education: Bachelors degree in Social Work, Psychology, Human Services, or a related field required. (Masters degree or additional certifications in the field are a plus.)
Experience: 35 years of casemanagement or related experience in the developmental disabilities field required. Experience should include working with adults in a day program or similar setting, with understanding of behavior management principles.
Knowledge/Skills: Familiarity with Individual Program Plan (IPP) processes and regional center or similar systems is highly desirable. Position Summary: The Family Engagement Coordinator position will support the Parent Partner Program in overseeing its direct implementation at Flynn Elementary School. This position will work directly with families, providing support for events, communication, resources, and additional information as needed at one school's site. The role will work with the partner school to outline the curriculum and desired outcomes for the programs.Key Responsibilities and Duties

Coordinate communication between the schools, after-school program staff, and families about resources and engagement opportunities available from the District and the community at large

* Conduct initial assessments and evaluations of students' mental health, behavioral, social and emotional problems and needs per Federal Special Education, Medi-Cal, and Board of Behavioral Sciences (BBS) requirements. Develop individualized mental health treatment plans that meet standards of clinical and ethical practice, referencing ERMHS and Individualized Education Program (IEP) goals and objectives. Maintain confidential client records per Health Insurance Portability and Accountability Act (HIPAA) regulations.
* Diagnose mental health conditions using the Diagnostic and Statistical Manual of Mental Disorders (DSM V) under the supervision of a Licensed Clinical Social Worker (LCSW).
* Complete daily progress notes and submit into the county- and district-adopted tracking and billing system for Medi-Cal billing. Bill for clinical services provided under Medi-Cal contract with Alameda County Behavioral Health Care Services.
* Participate on the Individualized Education Program team and prepare IEP documentation as required. Along with the IEP casemanager, develop social-emotional goals and objectives and positive behavior intervention plans for each student that are consistent with his/her individualized mental health treatment plan and Alameda County mental health goals, and report progress on those goals at intervals designated within the IEP.
* Collaborate with staff to develop an appropriate level of support and plan for each student. Provide consultation to personnel about mental health diagnoses, social-emotional needs and behavioral interventions.
* Facilitate individual, group, and milieu therapy services under the supervision of a LCSW. Provide casemanagement, family intervention, support, and education.
* Make referrals to appropriate outside agencies. Collaborate with outside agency providers, such as psychiatrists, psychologists, probation officers, and the Department of Social Services.
* Provide clinical crisis intervention services.
* Provide education and support to families about mental health diagnoses, social-emotional functioning and behavioral interventions.
* Provide education, counseling, and casemanagement services to support compliance with psychotropic medication prescriptions and on-going evaluation by the psychiatrist.
* Participate in monthly Peer Review/Quality Assurance meetings per Medi-Cal and BBS requirements.
* Prepare mental health charts for required six-month Clinical Quality Review per Medi-Cal and Alameda County Mental Health contract requirements.

* Graduation from an accredited four-year college or university, and an earned Master's degree in Social Welfare or Social Work (MSW) from an accredited college or university.
* Valid California Clinical Social Worker (LCSW) License or registered with the Board of Behavioral Sciences as an Associate Social Worker (ASW) currently in the process of accruing hours towards the examination process and licensure.
* Experience working with children or in an educational setting
* Valid California Pupil Personnel Services Credential with School Social Work authorization
* Valid California Driver's License, if applicable

The Social Worker must be a minimum of 21 years of age and pass all background clearances.
The Social Worker is responsible for the youth's admission, treatment, and transition process while at the STRTP. They provide evaluations, counseling, and education services to the youth and family. They also coordinate, lead interact with the youth's (CFTM) Child and Family Team Meetings and the (IPC) Interagency Placement Committee.
Responsibilities:
· Plan, coordinate evaluate and report on all required social work services for assigned youth.
· Complete an intake study for each youth.
· Complete a needs and (NSP) services plan for each youth and all required updates including mandated 30-day updates of plans.
· Obtain, develop and record data and information necessary for social work functions.
· Coordinate, evaluate, and report on the access to the provision of core services for each assigned youth.
· Create a transition plan and other documents as indicated.
· Provide individual, group, and family therapy to youths and their families.
· Participate in case conferences, Child and Family Team meetings, Interagency Placement Committee meetings, etc.
Social Worker will work to identify the services indicated in the Qualified Individual's (QI) assessment of the youth. When the QI arrives and conducts their assessment on behalf of the referring county, the QI will present their findings to the THYH Social Worker. The short and long-term goals for the youth from the assessment will be discussed with the QI, the authorized representative, and CFT members and incorporated into the youth's Needs and Services Plan (NSP) at that time.
Requirements:
Education and Experience:
Master's Degree from an accredited or state-approved graduate school, as defined by the Department of Education, in social work or social welfare, marriage, family and child counseling, child psychology, child development, counseling psychology or social psychology.
Completion of at least three semester units or 100 days of field practice or experience in a public or private social services agency with a master's degree or higher.
At least nine semesters units of course work related to children and families, or 18 months of experience working with children and families.
At least three semesters units working with minority populations; six months of experience working with minority populations; or six months in-service training working with minority populations within the first year of employment as a condition of employment.
At least three semester units in child welfare, or two years of experience in a public or private child welfare social services setting.
Must clear a CCL, DOJ, and FBI fingerprint criminal background check
Must have a valid California Driver License and be insurable under the Agency Insurance carrier
