  • Certified Nursing Assistant

    Assist nursing staff with patients' personal care, emotional support, and activities of daily living. More complex duties performed under supervision of licensed nursing personnel. ESSENTIAL FUNCTIONS: Performs patient care responsibilities considering needs specific to the standard of care for patient's age. Provides morning care which may include bed bath, shower or whirlpool, oral hygiene, combing hair, back care, changing bed linens, cleaning over bed table and bedside stand, straightening room and other general care as necessary throughout the day. Provides evening care which includes hands/face washing as needed, oral hygiene, back rubs, peri-care, freshening linen, cleaning over bed tables, straightening room and other general care as needed. Notifies appropriate licensed personnel when patient complains of pain. Prepares patients for meals; serves and removes food trays and assists with meals or feeds patients if necessary. Distributes drinking water and other nourishments to patients. Answers patient call lights, anticipates patient's needs and makes rounds to assigned patients. Assists patients with the handling of personal property: dentures, glasses, contact lenses, hearing aids, prosthetic devices, etc. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Transports patients to and from various departments in the hospital. Reports and, when appropriate, records any changes observed in condition or behavior of patients and unusual incidents. Assists in the preparation of admissions, transfers and discharge of patients. Assists licensed nursing staff with treatment procedures. Demonstrates ability to revise plan of care as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness. Integrates the plan of care with Rehabilitation Services treatment plan to attain patient's optimal level of functioning. Provides general nursing care: positioning patients, lifting and turning patients, applying/utilizing special equipment, assisting patients with the bedpan, commode; assists ambulating patients. Uses correct body mechanics. Checks patient for reddened areas or skin breakdown and reports to licensed nursing staff. Obtains and records temperature, pulse, respirations, blood pressure, weight, intake and output. Makes rounds with oncoming shift to ensure the unit is left in good condition. Maintains a good working relationship both within the department and with other departments. Communicates appropriately and clearly to nurse, supervisor, co-workers, physicians and the Rehabilitation Services staff. Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient's needs. Demonstrates an ability to be flexible, organized and function under stressful situations. Treats patients and their families with respect and dignity; ensures patient confidentiality. Practices careful, efficient and non-wasteful use of supplies and linens and follows established charge procedures for patient charge items. Follows infection control procedures. Keeps the unit clean and organized. Meets current documentation standards and policies. Manages and operates equipment safely and correctly. Response to the patient is appropriate in emergency or physically distressful situation. Performs other duties as assigned within the nurse assistant's scope of practice. Other duties as assigned Qualifications QUALIFICATIONS: Completion of course for nursing assistants Current state certification as a Nursing Assistant CPR/BCLS Certification Minimum six months experience in an acute or long term facility preferred. Excellent oral and written communication and interpersonal skills.

    POSITION PURPOSE : To provide direct patient care and management of adults (ages 18+) as prescribed by the physician and treatment team; to provide medical management and assessment as prescribed by a physician; to provide safety and crisis management for patient populations. KEY RESPONSIBILITIES: To provide leadership and direction to Psychiatric Technician. To assess patient care. To administer medication and medical treatment as prescribed by a physician. To provide and direct milieu management. To document nursing care and treatment progression. Implement nursing and milieu care. Initiate and account for special treatment procedures. To assess, direct, and intervene in crisis situations. CUSTOMERS SERVED: Patient, families, physicians and other clinical staff, nurses and psychiatric technician. PERFORMANCE STANDARDS: Weight Performance Standard 1. 5% Receives shift report from oncoming shift; Prepare and give outgoing shift report. 2. 5% Completes unit/staff assignments assuring that team members are competent to complete assigned tasks and have the information necessary to assure a safe and therapeutic environment. 2. 5% Completes unit/staff assignments: (continued) Discuss assignments with team members to clarify expectations for: Patient monitoring during on and off unit activities Patient care assignments Code assignments Staff breaks. Assign patients to staff members, keeping in mind staff competence level, patient acuity, gender, and precautions. Supervise staff completion of assigned tasks. 3. 5% Complete initial environmental safety rounds at the beginning of each shift to ensure that any safety issues are addressed at that time (e.g., all patients are present and environment is safe, etc.) 4. 5% Check charts for Physician orders at beginning and end of shift to ensure that all orders have been addressed. Review and chart vital signs or make assignment and follow up to ensure that task is completed effectively. 5. 5% Write ongoing Patient Assessments which reflect the full range of patient needs including physical, functional, psychosocial and safety needs. Write documentation which reflects initial status, changes and progress in all areas. 6. 5% Support therapeutic programming on the unit by expecting that the schedule is followed and that all therapeutic groups begin and end at the assigned time. 7. 5% Transcribe Physician Orders as they are written or make assignment and follow up to ensure that task is done effectively and accurately. Give reports to Physicians on their patients when they make rounds or make assignment and follow up to ensure that task is done effectively and accurately. 8. 5 % Completes nursing portion of Discharge Plans and reviews it with discharging patients. 9. 5% Administer medications according to departmental procedure and physician's orders. Ensure that medication labels are checked for expiration date and correctness of directions. Ensure that patients receive medication within one hour of prescribed time. Chart all medication administered, documenting any adverse reactions and responses to PRN medications. 10. 5% Serves as the Dr. Strong Code Leader for the unit. This includes being able to identify the beginning of a crisis and to intervene early to avoid crisis as well as taking a leadership role and following Dr. Strong Code Protocol when a Dr. Strong Code is called. 11. 5% Completes admission nursing assessment and Initial Nursing Treatment Plan of the patient within 8 hours. Participate in the interdisciplinary treatment team by attending staffing and/or documenting on the treatment plan and implementing the nursing process. Work with Treatment Team (Social Services, Physicians, Recreation Therapy, etc.) to ensure that Treatment and Discharge Planning is complete and accurate. 12. 5% Complete AED, glucometer and refrigerator checks or makes assignment and follow up to ensure that task is done effectively accurately. 13. 5% Treats others with dignity and respect at all times including patients, family members, and fellow staff. Demonstrate honesty, integrity and ethical courteous behaviors. 14. 5% Accept and respect individual differences (Culture sensitivity and diversity) among patients, family members and fellow staff. 15. 5% Is flexible, teachable, willing to accept change and discuss alternatives in a positive manner. Accept constructive feedback and implement active measures to make improvements. 16. 5% Adheres to company policy and procedure related to designated work hours, breaks, schedule and attire. Follow protocol for requesting time off and unexpected absences.
