Supports residents in daily living decision making regarding care and services in accordance with established service plans. * Maintains and completes required documentation. * Provides clear, timely communication related to changes in health, emotion and behaviors to direct supervisor. * Meets educational, training and competency requirements on time including SAMMS. * Responds to emergency situations in a prompt and calm manner and provides basic first aid care when needed within the limits of training and the policies of The Loomis Communities. Ensures that an individualized, updated service plan is in place to meet resident needs and reviewed every six months, with resident using information gathered from clinical assessments, resident or family input and RA communications. Plans and implements engagement opportunities with residents offering programs for residents who may be at risk for isolation or decline, including but not limited to residents living with cognitive changes or physical abilities. Schedules programs based on residents' daily routines and preferences. Meets with residents to establish connections, understand past and current preferences for engagement and when applicable, meets with family members to gain additional knowledge of residents. Ongoing evaluation of programs offered at Loomis Village to ensure residents have the opportunity to access programs and participate based on interest and abilities. Promotes a culture of hospitality, inclusion and wellness. Completes ongoing educational opportunities and trainings to enhance resident engagement. Completes interest surveys including daily routines and past interests with residents and family members as needed. Creates programs based on the survey information to promotes the individual needs, preferences and interests of residents using a resident directed approach. Engages with other team members on ways to collaborate with joint programs. Identifies and implements volunteer opportunities. Assists in departmental training, meetings, events, and new team member/ resident onboarding. Works with the director and assistant to research collaborate on new program ideas, including shared programs with the external community. accurate and timely communications, including monthly calendar accuracy, and contributes to the development of key performance indicators based on the wellness strategic pillar and successful living. Qualifications Preferred Education A Bachelors degree in therapeutic recreation or a related social or human services field. Preferred Experience A minimum of 1 year of related experience.
