Job Description
Compiles, verifies, and scans medical records of Lowell Community Health Center by performing the following duties:
• Verifies all paperwork is completed at well as Practice Management of newly acquired patients
• Reviews medical records for completeness, scans records in designated areas within EMR
• Attaching Labs & X-Rays in EMR, and to medical documents
• Answer phones/check and respond to voicemails in a timely and courteous manner
• Attend to patient and department requests. (Mailing, Faxing, Correspondence, and Patient Requests)
• Excels in customer service
• Detail Oriented
Qualifications
• High School Diploma or equivalent
• General Office Skills: typing, phone management, and communication skills
• At least 6 months office experience with a strong customer service background.

Job Description
The Pediatrics Department Manager is responsible for ensuring the delivery of safe, effective and culturally appropriate care and services to our patients. The manager will work collaboratively with medical staff and other departments to ensure that the goals of the Health Center are accomplished. Responsibilities include, but are not limited to: Provide excellent customer service to all patients and staff; Develop and update departmental standard operating procedures; Manages the organization's pediatric immunization inventory levels; Coach, mentor and teach staff to ensure that they performing to their highest potential;
Provide timely and relevant performance evaluations to staff in accordance with LCHC policy and supervisor expectations; Works with clinical lead to manage clinical workflow within the department; Work with departmental supervisors and leads to create and maintain appropriate staff and provider schedules; Establish an environment which fosters teamwork and open communication; Ensures that care coordination activities are being performed including obtaining documentation such as follow up of hospitalizations, ER visits, and other medical and behavioral documentation; Oversee the budget development and management of the department; Responsible for ensuring department achieves or surpasses visit volume and revenue projections.
Qualifications
The Department Manager will possess a Bachelor's degree in a healthcare/public health related area or equivalent years of experience. Job Description
The medical interpreter serves as an interpreter for patients and staff and supports the operations of the department. The interpreter uses, consecutive, simultaneous, and sight translation modes. Relays medical information between speakers of two different languages in compliance with all office and Lowell Community Health Center (Health Center) policies and procedures, particularly relating to patient confidentiality and informed consent, passing performance of competencies listed in the International Medical Interpreter Association (IMIA) Standards of Practice, and the Code of Ethics.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Helps facilitate successful delivery of services to linguistically diverse patients.
Explains Health Center resources, office protocols, and limitations to providers, clinicians, and patients; makes appropriate referrals; serves as a cultural and linguistic resource to both patients and providers to contribute to successful outcomes.
Performs duties in a spirit of teamwork and cooperation.
Adheres to Health Center policies on customer service.
Understands the Health Center's approach of service delivery.
Completes written translations as required in patient encounters such as filling out forms and medical instructions, and medication schedules; administrative translations such as letters to patients, documents, and patient materials.
Supports the office operations by answering the phone, taking requests, calling patients to confirm appointments, documenting encounters on the interpreter tracking system, entering data, verifying and scheduling future appointments when necessary, general troubleshooting, filing, and photocopying.
Utilizes on-line computer systems such as Outlook, the Health Center's Electronic Health Record, and other Health Center software.
Works with special projects and other duties as assigned.
Participates in staff meetings and completes necessary departmental reports
Qualifications
QUALIFICATIONS:
Fluency in English and Portuguese, both oral and written, with a minimum of Advanced Mid proficiency level as evidenced through language assessment test results from a recognized body such as ******************************************************************************************
Ability to work accurately in consecutive mode and sight translate into and from working/ target language(s).
Ability to use both languages fluently and accurately on all levels pertinent to professional needs.

The Health Benefits Advisor serves as the Health Center's primary contact for patient needs such as assisting patients in completing MassHealth and Health Insurance Assistance Program applications; providing information to insured, underinsured and uninsured individuals about coverage and about alternate financing and other financial assistance programs that may be available; and acting as an administrative resource to the health center and or billing staff when problems or questions with eligibility arise.

The Outreach and Enrollment Coordinator serves as the Health Center's community contact for consumers enrolling in and maintaining access to coverage

Job Description
As a central member of the patients' multidisciplinary heatlh care team, the Complex Care Manager (CCM) will be a Registered Nurse who identifies and engages patients with a focus on improving patient experience, improving health and reducing costs. Care management of complex patients is the deliberate organization of patient care activities to facilitate optimal delivery of health care services. Organizing care involves a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs. This is managed by the effective exchange of information among all participants on the care team, including the patient. Complex care management involves the oversight and coordination of care delivery targeted to high risk patients with diverse combinations of health, function and social problems. Excellent interpersonal skills, patient engagement skills and the ability to work indenpendently and collaboratively are key requirements of the role.
Provide care management services for highly complex patients within the health center. Utilizing a high-risk patient registry and Primary Care Provider (PCP) input, the Complex Care Manager will identify the patients, perform risk stratification and conduct comprehensive assessments. The CCM will then work with the care team, and also independently, to develop a comprehensive, proactive, evidenced-based integrated care plan (ICP). The CCM will be responsible for managing an active case load, as well as maintaining a registry of complex patients.The Complex Care Manager is responsible for working with patients to identify strengths and barriers and develop an individualized, patient-centered plan of care.

responsibilities in an outpatient setting. This position helps promote and maintain a well-coordinated and efficient operation in an atmosphere that promotes a positive and professional experience for our patient population. The OBAT SUD Recovery Coach performs a variety of clinical support functions to facilitate the smooth flow of operations.
As an integral member of the patient care team, has a special role in working closely with the providers in their relationship with the patient and facilitates contact and support with the patient following referral to the OBAT Program.
The Recovery Coach works with the OBAT clinical care team to ensure that the goals of the Health Center mission are accomplished. The Recovery Coach, he/she will provide recovery support services which will assist patients in achieving sustained recovery.
This position will facilitate patient access to recovery-oriented SUD treatment, resources and community recovery groups. The recovery Coach will bridge treatment systems into a more holistic care model through providing support across multiple systems of care. The recovery support services designed to generate individualized recovery options and assist patients to remove challenges to/of recovery.

SUMMARY:
Full Time (40 hours per week). Some evenings and weekends required, inclusive in the 40-hour work week. The Care Coordinator/Community Health Worker (CHW) will be an integral member of an interdisciplinary Behavioral Health and Primary Care Integration team in Pediatrics and Family Medicine Departments. The CHW will work closely with patients, care managers, other care management team members, health care providers, behavioral health and social services providers, and community partners to effectively manage the care of designated patients.

Job Description
Process incoming and outgoing referrals. Responsible for scheduling specialty appointments for health center patients and informing patients of date, time and location of appointments. Respond to inquiries from patients, staff, internal and external physicians, hospitals, other medical groups and third party carriers. Maintain data and referral tracking system. Work collaboratively with other members of the health care team in order to provide superior patient care delivery. Provides overall excellent customer service.
Other responsibilities include:
• Process incoming and outgoing referrals sent via the electronic medical record (EMR) from the medical providers.
• Upon receiving request for referral, verify insurance coverage and obtain approval as needed.
• Research and ensure that the specialist referring to accepts the patient's insurance plan.
• Contact specialty office and schedule appointment for patient.
• Assemble referral paperwork and required medical information and submit to specialty office as needed.
• Respond to providers via telephone encounters and actions within EMR when additional information is needed.
• Ensure that all required forms are obtained, completed and signed.
• Send referral and any pertinent information to patient regarding their scheduled specialty appointment. • Respect and maintain privacy and dignity of patients/clients: assure patient/client confidentiality at all times
• Interact with patients on site and via phone regarding questions about referral appointments.
• Provide excellent customer service to both external customers and internal customers
• Ensure that referrals are processed in a timely manner in accordance with health center policies.
• Participate in staff and educational meetings/trainings
• Maintains current knowledge of referral processes, health center systems and changes.
• Where needed, provide cross -coverage and back-up staffing support to other functions of the Patient Service Center, including Health Benefits, Greeters and Registration services.
• Perform other work-related duties as assigned
Qualifications
High school diploma or equivalent, two years office experience in a health care setting preferred. Knowledge of medical terminology required, medical assistant certification is a plus. Knowledge of health care system and insurance eligibility is a plus. Must have effective communication skills both verbally and written. Data entry, computer and phone skills are required.
Bi-lingual in English and Spanish, Portuguese or Khmer preferred.

Job Description
Serve as a Health Educator/Case Manager for the Teen BLOCK teen pregnancy prevention programming including recruitment, facilitation, training, program delivery, and counseling youth.
Foster the development of at-risk youth from the ages of 13-18 with a focus on youth from Latinx cultural backgrounds.
Assist with coordination of program including meetings, setting agendas, mentoring peer leaders to engage in meeting facilitation, and coordinating follow-up.
Attend all appropriate trainings and weekly program meetings.
Assist program members with health care, testing and treatment referrals and educate members on these processes.
Support participants to develop skills and knowledge necessary for positive decision-making and supportive of themselves and others.
Provide individual case management, counseling and support of participants with support from Clinical Supervisor.
Assist with required data collection and evaluation reporting as directed.
Collect input from parents, teens, community residents assuring target population representation in program decision-making processes and direction of outreach and education.
Develop and maintain a positive working relationship with collaborating schools and service providers, including other Lowell Community Health Center Departments.
Provide evaluation support to the Teen Pregnancy Prevention Coordinator
Maintain standards that ensure quality of care in the delivery of program services and compliance with state and federal guidelines.
Qualifications
Required:
Community organizing skills.
Excellent verbal and written communication skills.
Strong knowledge of prevention work, particularly sexuality education prevention.
Strong presentation skills
Two to four years' experience in an adolescent service setting or health related position
Demonstrated experience working with diverse cultures.
Bilingual in Spanish and English required.
Must have reliable transportation.
Proficient computer skills in MS Office are required.
Preferred:
Bachelor's Degree in Public Health, Health Education or related field
Community Health Worker Training Certificate
HOURS: Monday - Thursday 8:30am - 7:00pm
Flexible nights and weekends.
