    Job DescriptionDescription: The Case Manager - School-based Intervention Services will identify the needs of the middle school students enrolled in programming and their families and connect them to community resources to meet those needs. This position will meet with students and families on a regular basis both at school and within their homes. This position will gather student/family information via screenings, assessments, and consultation with collateral sources. With the student/family, the Case Manager 1 will develop goals for an Individualized Action Plan. Ratings on progress toward these goals as well as discharge planning and implementation are expected. The Case Manager 1 will mentor students and will work on a team with other RISE staff to plan and lead activities and psychoeducational groups for students and their families. The qualified candidate will have a Bachelor's degree in Human Services, Counseling, Social Work, Psychology or closely related field. A minimum of 3 years of related experience preferred. Knowledge of behavioral health, violence prevention/intervention, positive youth development, and cultural responsiveness preferred. Group facilitation, strong relationship-building skills required. Excellent communication and computer skills required. Valid driver's license and own transportation required. Skills Counselors implement recreational and skill building activities with youth in accordance with their individual treatment plan as well as the program model and philosophy. Skills Counselors are charged with maintaining the safety and well-being of youth, in part by maintaining a calm, trauma-informed therapeutic milieu and by providing a safe, clean, home-like environment. Both staff and residents assist with the daily upkeep of the residence. Skills Counselors implement and actively participate in the activity schedule with the goal of encouraging adaptive behaviors and improved social skills. Skills Counselors assist and teach residents the skills necessary for self-care and activities of daily living. At times, Skills Counselors transport youth to school, medical appointments, and other places. Many of our residents take prescription medications regularly. It is the responsibility of the Skills Counselor to participate in medication management for youth. Skills Counselors manage crises that may arise, with the support of the staff team. It is imperative that staff use appropriate language and tone when setting expectations and limits for clients. All staff must maintain appropriate therapeutic and professional boundaries with residents. Throughout all tasks, timely and accurate documentation is required. Staff must comply with all contractual, licensing, and safety standards and maintain current certifications for CPR/First Aid and CPI - non-violent crisis intervention. Training is provided. The qualified candidate will preferably have an Associate's degree in a human service field and 1-5 years of experience in a residential setting.
