Key Responsibilities:
Conduct initial and ongoing comprehensive assessments of patients, including OASIS documentation.
Develop, implement, and evaluate individualized care plans based on patient needs.
Administer skilled nursing care such as wound care, IV therapy, catheter care, medication administration, and chronic disease management.
Monitor patient progress and communicate changes to the physician and care team.
Educate patients and families on disease processes, medication management, and self-care techniques.
Document all visits and interventions accurately and timely in the electronic medical record (EMR).
Ensure compliance with all applicable federal, state, and local regulations and company policies.
