HOW YOU WILL MAKE AN IMPACT:
The multilingual (English/Spanish) Administrative Assistant supports clients and team members by providing phone support, reception services, reception and direction of clients and visitors, scheduling client appointments, and providing administrative support to District partners.
Frequently communicate via reading, writing, speaking and translating in Spanish and English.
Remain aware of the lobby, welcome and direct clients and visitors to the appropriate person.
Answer and route incoming phone calls to proper individuals and routinely check phone messages and route or return calls the same day unless the message was left after hours.
Schedule appointments for a variety of our programs
Create client profiles in various systems and databases
Verify client insurance and accept/collect required documents.
Receive registration for client course (ex: Citizenship classes)
Support client referrals, service coordination with PCP's
Accept payments from clients and post to BlueDot
Other administrative duties as assigned

HOW YOU WILL MAKE AN IMPACT:
Case Manager/Parent Coach will work with families who are involved with the ODHS Child Welfare system and/or reside in the Transitional Treatment Recovery Services program. Parent Coach will provide varying hours of hands-on parent coaching/education, interventions, and skills training to achieve behavioral changes in families where child safety issues are present. Provider is identified as a "Safety Service Provider" and will provide transportation and services in the community and in the family's home. Will attend meetings and collaborate with ODHS Caseworkers, community partners and other agencies, which includes providing detailed and timely oral and written reports. Parent Coach will also use evidence-based curriculum to implement and facilitate parent education groups in a community living environment.
Provide a range of services in-home or community based, including parent education, modeling and mentoring, communication skills, self-care, home management, nutrition, etc. to support and achieve behavioral changes in families where child safety issues are present.
Assess parental capacities related to child safety, well-being, and needs to help resolve child and family safety and attachment concerns
Identify and create goals in partnership with ODHS Child Welfare and client
Keep accurate records and communicate effectively orally and in writing to document clients' progress and to gather and exchange information
Provide supervised visits to children separated from one or both parents to ease the transition from foster care or relative placement to home
Provide respite care services in the home or community, including recreational activities and behavioral monitoring
Prepare curriculum and teach parenting groups as needed
Work a 24/7 flexible schedule to meet families' needs and be available for crisis consultation: including occasional weekends and evenings, and be available for on-call rotation
Coordinate access and provision of services from multiple agencies, including establishing service linkages, advocating for service needs and obtaining entitlements
Work with a variety of individuals in difficult circumstances, including angry and hostile clients, disadvantaged individuals, families in provision of services, those dealing with mental/emotional health challenges, and clients with developmental delays.
Identify families with drug and alcohol dependence, physical, and sexually abused children

The Family Peer Support Specialist offers emotional and adjustment support, along with case management services, to both LCSNW clinical participants and community members. As an important member of our interdisciplinary team, the LPN delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards. Your role will involve monitoring and reporting changes in the patient's condition. On a daily basis you must be able to assign duties, instruct, and provide clinical supervision to CNAs.
Education, Experience, and Licensure Requirements
Graduated from an accredited LPN program
Currently licensed/registered in applicable State. Must maintain an active Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing throughout employment.
One (1) year skilled nursing experience preferred
CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

HOW YOU WILL MAKE AN IMPACT:
The Home Visitor will provide parenting information, resources, and support to highly stressed parents as a member of the Relief Nursery team at A Family Place. This involves providing intensive home visiting services, group facilitation as needed, and also being a lead staff member in providing regular respite opportunities for families. Spanish speakers are strongly encouraged to apply. This is a full-time Position at 1 FTE (37.5 hours per week) Which requires the ability to work some evening hours.
Work collaboratively with staff, interns, volunteers, and community partners to support the mission of keeping children safe and families together.
Maintain a caseload of 15-20 families, ensuring complete and accurate documentation, including releases, medical information, and emergency contacts.
Conduct home, community, and site visits, which may include transporting families to and from community resources (in personal car) and AFGP events; assess child development using evidence-based tools, provide resources/referrals, and deliver crisis support when needed.
Communicate developmental goals and progress to families, model positive interactions, and promote problem-solving skills.
Provide regular planned respite care as lead teacher and ensure inclusive practices for children with disabilities.
Advocate for families and coordinate services, including representing AFGP at community and IFSP meetings.
Apply principles of diversity, equity, cultural sensitivity, and trauma-informed care in all family interactions.
Participate in professional development and supervision, attending meetings, trainings, and weekly reflective supervision sessions with the Program Supervisor.
HOW YOU WILL BE A GREAT FIT:
1 year of related experience working with families with multiple risk factors and knowledge of the community.
Associate's / Bachelor's / master's in early childhood or related field
ORO Step 9 or bachelor's degree in early childhood or education. Credentials may be required depending on individual program or location requirements.
Fluency in English, both spoken and written
Basic mastery of business applications and productivity suites

Travels between worksites regularly; responsible for transporting clients in personal and/or agency vehicles, which requires valid driver's license, auto insurance, and motor vehicle records clearance
Works in a variety of settings, including and not limited to LCSNW office locations, partner agency and community-based settings, client home/community settings, outdoor settings in varying weather conditions, and faith-based settings such as churches
Meets LCSNW criminal background clearance requirements and other job-relevant clearances as required by the State or program, must be enrolled in the Central Background Registry (CBR), Oregon Dept of Early Learning and Care Registry
