Education/Training:

General Responsibilities

Essential Duties
* Supports the mission and values of Lutheran Social Ministries of New Jersey, which is: Through the power of the Holy Spirit, and in response to Gods love as revealed in the Gospel, the mission of the Lutheran Social Ministries of New Jersey is to serve those in New Jersey who hurt, are in need, or have limited choices
* Under the supervision of the Home Care Coordinator and the direction of the Director of Nursing, the Scheduler for In Home Services will be responsible for the coordination and scheduling of in-home services for all Lutheran Senior LIFE PACE participants.
* Schedules in-home service staff appropriately to meet participant needs.
* Prepares schedules according to participant needs, Director of In-home care requests, and within the plan of care designed by the interdisciplinary team.
* Ensures that all required personal documents are forward to the center prior to the first home care assignment.
* Communicates staffing needs and ability to meet participant needs with Home Care Coordinator.
* Coordinates and communicates effectively with the transportation department to ensure smooth transition between home care and transportation delivery of services
* Notifies participants/families of changes in their home care schedules.
* Prepares and maintains daily, weekly, and/or monthly service schedules, logs, ledgers, tickler systems and other records that ensure accurate data collection of home care services.
* Communicates clearly with participants, staff and supervisor directly and in meetings.
* Assists with filing and other office functions as requested.
* When authorized, communicates with outside agencies and independent contractors regarding services rendered on behalf of Lutheran senior LIFE.
* Attends Intake and Assessment meetings if requested by Interdisciplinary Team or supervisor.
* Responsible for the ordering and distribution of home care supplies for the participants.
* Responsible for the ordering of office supplies for the home care department
* Attends all departmental meetings.
* Reports pertinent participant events or missed participant in home service visits to the Home Care Coordinator.
* Follows all Lutheran Senior LIFE Policies and Procedures and OSHA safety guidelines.
* Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
* Practices Universal precautions and follows appropriate Infection Control procedures.
* Maintains a safe working environment. Follows Lutheran Senior LIFE Safety Policies and Procedures.
* Participates in continuing education classes as assigned and any required staff and training meetings.
* Maintains professional affiliations and any required certifications.
* Treats employees, patients, and business partners with respect. Fosters teamwork by trusting and supporting each other while encouraging collaboration at all levels of the organization. Embraces diversity and demonstrates the ability to work together.
* Promotes a safe and efficient working environment by adhering to agency policies and procedures.
* Maintains confidentiality of all agency and patient related information. Protects patient rights as defined in the patient Bill of Rights.
* Maintains proper attendance and punctuality to ensure that the department is operated in an efficient and cost effective manner.
* Adheres to departmental and agency dress code outlined in the organizations policy.
* Review and approve timesheets for Home Health Aide.
* Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.
* Adheres to the Organizations Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
* Follows all local, state and federal regulations as they pertain to the position.
* Adheres to the Organizations Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
* Other duties as assigned by my supervisor necessary for the efficient operation of the department/facility.

Must demonstrate skills and have a passing score on a written competency test in accordance with 42 C.F.R. Section 484.36 or 418.76(c) prior to working independently with a patient.
Valid drivers license and reliable transportation that is insured in accordance with New Jersey requirements.
Skill(s): Must maintain a professional attitude and demeanor and is be able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers and all others.
Must be able to demonstrate optimism, enthusiasm and willingness to work constructively with other health care team members and reflect concern for the well-being of patients and employees.
Interact with all staff in a positive and motivational fashion supporting the mission and vision of the organization.
Experience: Minimum age requirement of eighteen (18).
One (1) year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred. General Responsibilities
Under the Supervision of the Medical Director, the Nurse Practitioner provides primary care to participants. Performs physical assessments of new enrollees, semi-annual reassessments of participants and develops and implements appropriate plans of care. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Demonstrates the knowledge, skills, and abilities necessary to assess, plan, care for, and provide services to frail and elderly participants according to assigned responsibilities and Lutheran Senior LIFE standards.
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact

Provides leadership to Property Managers, directs the certification process; ensures compliance with all certification and rental agreements and all regulatory agencies.
Supervises Property Managers of facilities throughout the State; ensure that assigned duties of Managers are performed effectively in compliance with agency policies and procedures.
Sets performance standards for staff related to quality, compliance and resident services; communicates expectations to staff, residents and the community.
Develops budgets for each facility and the Housing Management Department and is responsible for adherence to financial goals and objectives.
Assist in negotiations for purchase of properties and financing terms.
Work with CFO in performing financial analysis to determine financing strategy for potential projects. Prepare and review all materials for housing development financial closings.
Ensures that occupancy targets are achieved.
Establishes and maintains effective relationships with various LSMNJ internal constituencies and other external regulatory agencies in or to effectively meet operational needs.
Select and oversee professional service consultants (architects, engineers, etc) managing architectural design activities, pre-construction and construction.
Attend construction meetings as owner representative with all related parties.
Identifies, develops and implements new business opportunities for potential housing development, commercial developments, community revitalization and/or other program initiatives for LSMNJ.
Oversees social services and other resident service programs.
Ensures good physical plant through building maintenance, preventative maintenance programs and capital improvements.
Assist and participate in Year 15 re-structures.
Develops, implements and maintains Housing Management policies and procedures, as well as standard operating procedures for the properties.
Prepares information as needed for the Board of Trustees Housing Committee and LITCH Boards. Schedules meetings and takes minutes as required.
Assists with other facility development issues in other programs as requested.

* Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.
* Adheres to the Organizations Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
* Follows all local, state and federal regulations as they pertain to the position.
