This position offers flexibility to work with our clients. General Responsibilities: This is a professional position responsible for the provision of a wide range of psychosocial services to terminally ill patients and their families, including completing the comprehensive psychosocial assessment and regularly assessing the patient and family psychosocial needs on an ongoing basis. The social worker also completes the bereavement risk assessment of the family and provides counseling relative to anticipatory grief issues. Upon completion of the assessment, a psychosocial plan of care is developed with the Interdisciplinary Team (IDT) and the patient/family. It is the role of the social worker to implement and continually revise and reassess this plan relative to the psychosocial and potential bereavement issues of the patient and family. The social work services provided may include but are not limited to: supportive counseling, problem solving and referrals to community resources, pre-bereavement counseling and support. Education/Training/Certifications: * Has a Masters Degree in Social Work (MSW) from a graduate school of social work accredited by the Council on Social Work Education (CSW) * Must possess current license as a Social Worker by the Pennsylvania State Board of Social Work Examiners. * Must be a licensed driver with an automobile in good working condition and have automobile insurance in accordance with Pennsylvania laws. Skill(s): Skills and Knowledge: * Visual/hearing ability must be sufficient to comprehend written/verbal communication. * Excellent verbal communication skills are needed. * Able to effectively manage stress. * Manual dexterity sufficient for writing, keyboarding and telephone operation. * Able to work in a variety of settings and conditions in patients homes and facilities. Experience: * Understanding of Hospice philosophy, principles of death/dying. * Hospice or home care experience desired. * Ability to assess patient needs and formulate individualized patient care plans to meet those needs. * Ability to work in a field setting and exhibit ability to make sound judgments. * Must have effective communication, comprehension, documentation, interpersonal and computer skills. Select and oversee professional service consultants (architects, engineers, etc) managing architectural design activities, pre-construction and construction. Oversees social services and other resident service programs. Ensures good physical plant through building maintenance, preventative maintenance programs and capital improvements.     Looking for a fulfilling experience? Journey Hospice is seeking Volunteers for our program! We are looking for friendly visitors to spend time with our hospice patients. All you need to do is talk, listen to music, play games and spend quite time together. This is a great opportunity to share a musical talent or pet therapy. Volunteers need in Philadelphia! Training and mileage provided. The minimum age of volunteer is 18. The training consists of a mix of classroom and self-study. Each volunteer meets with the volunteer coordinator to discuss topics of compliance, how to visit patients and families, documentation and what to expect when working with hospice patients. The self-study consists of reading and watching videos dealing with topics such as dementia, communication, spirituality, and the dying process. The goal is for each volunteer to feel comfortable when they visit. Journey Hospice does require a flu shot (between Oct March), COVID vaccination, background clearance and drug testing.

Qualified candidates MUST have reliable transportation and be willing to drive to both home and facility-based patients. This position offers flexible scheduling, covering 5 days/per week. The Hospice CNA provides personal care and light housekeeping services to terminally ill patients in the home, nursing facility, inpatient units or other environments where the Hospice patient resides in accordance with an established plan of care and under the supervision of a Registered Nurse (RN). Actively participates as a member of the Hospice Interdisciplinary Team (IDT). Provides care and comfort to patients in accordance with assigned tasks and established interdisciplinary and Hospice Aide plan of care. Observes the patient, reports and documents observations and care performed. Provides timely documentation of services based on the patients care plan Basic Qualifications: Education/Training/Certifications: Must have High school diploma or GED. Active and in good standing Pennsylvania Certified Nursing Assistant (C.N.A) required. Must demonstrate skills and have a passing score on a written competency test in accordance with 42 C.F.R. Section 484.36 or 418.76(c) prior to working independently with a patient. Valid drivers license and reliable transportation that is insured in accordance with PA requirements. Skill(s): Must maintain a professional attitude and demeanor and is be able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers and all others. Must be able to demonstrate optimism, enthusiasm and willingness to work constructively with other health care team members and reflect concern for the well-being of patients and employees. Interact with all staff in a positive and motivational fashion supporting the mission and vision of the organization. Experience: Minimum age requirement of eighteen (18). One (1) year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred.
