As a Care Companion you will:
Complete program outreach, friendly visits (telephonic and in-person), light household chores, and personal errands (such as grocery shopping and transportation).
Document direct care visits of members in service, assuring compliance with program regulations, and managing relationships with multiple stakeholders in the community.
Provide provision of direct client services, including LSS Healthy Transitions and similar services, under the supervision of a certified staff member.
Qualifications & Requirements:
Ability to work well with diverse individuals, families, groups and organizations and demonstrate compassion, patience, flexibility and respect for others.
Provide regular in-home/telephonic visits to members and provide transportation in the community as needed.
Have dependable transportation and be willing to transport members in your vehicle.
