As a Care Companion you will:
Complete program outreach, friendly visits (telephonic and in-person), light household chores, and personal errands (such as grocery shopping and transportation).
Document direct care visits of members in service, assuring compliance with program regulations, and managing relationships with multiple stakeholders in the community.
Provide provision of direct client services, including LSS Healthy Transitions and similar services, under the supervision of a certified staff member.
Qualifications & Requirements:

Provide regular in-home/telephonic visits to members and provide transportation in the community as needed.

You will supervise and monitor client personal care and health care, monitor and complete court required documents/forms and be on-call for client emergencies. As a substitute decision maker, you will manage client financial affairs, secure and oversee financial assets/accounts (checking, savings, CDs, stocks and other investments) and maintain client files and a detailed activity log of all services and activities performed on behalf of the client. You will also monitor, guide and supervise the guardians/conservators, ensuring the quality of services provided, provide interim supervisory services state-wide as needed and ensure the volunteer program is monitored and supported regionally.
As a Supervising Guardian you will conduct educational & informational presentations regarding program services, develop and maintain contacts with other service providers in the community (legal, court/government, medical and human services), suggest and make referrals to other service providers when appropriate for a specific client and attend and participate in regular meetings with nursing home staff, group home staff, work site staff and county personnel. You will also be responsible for maintaining updated knowledge of the MN statute for Guardians/Conservators, LSS policies and procedures, MinnesotaAssociation for Guardianship & Conservatorship (MAGiC) and National Guardian Association (NGA) Standards of Practice to ensure the integrity of services provided and adherence to applicable law. You will review referrals to LSS Guardianship Options and assess situation to determine they are accepted or declined, maintain the client report system and conduct client outcome assessments.
Job Requirements:
* Bachelor's degree in human services or other relevant field plus two years of experience with similar client populations and needs or 6 years of experience in a closely related field with a similar client population and in a similar decision making capacity.
* One year of supervisory experience is desired.
* Specific training in the area of social work practices, public relations, legal procedures and vulnerable adult legislation is desired.
* A valid driver's license, acceptable driving record and a Motor Vehicle Check.
* Successfully complete the requirements of a background check.
