About the Patient:
We are seeking a compassionate and experienced overnight nurse or caregiver to provide in-home care for a 13-year-old boy living with Nemaline Myopathy (a condition that causes muscle weakness) and Arthrogryposis Multiplex Congenita (which affects joint flexibility and positioning).
He is ventilator-dependent via a tracheostomy and receives all nutrition and medications through a GJ tube. He uses an eye-gaze computer to communicate and requires a wheelchair for mobility. Due to severe osteoporosis and joint contractures, all care must be performed with an extremely gentle and attentive approach.
Primary Overnight Responsibilities:
Monitor ventilator and pulse oximeter throughout the night
Perform tracheostomy suctioning as needed
Provide tracheostomy cleaning and routine care
Administer GJ tube feedings and occasional tube medications
Use cough assist machines to manage respiratory secretions
Administer respiratory treatments including nebulizers and inhalers
Assist with bedtime hygiene routine including showering

Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.

Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. CNA training and certification required and provided by facility.
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department specific required certifications.
When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor

Job Title: Pharmacy Clerk Reports to: Chief Pharmacist, Staff Pharmacist Status: Non-Exempt The Pharmacy Clerk's primary responsibility is patient/customer care. This position is the first point of contact for pharmacy patients and will handle all patient/customer interactions in a prompt, professional and friendly manner.
Essential Duties and Responsibilities
* Learn Lewis Drug policies and procedures and perform them daily
* Professionally manage and assist in all patient wait times, sales, returns, and complaints
* Answer phone calls promptly and in a polite manner
* Obtain and maintain pertinent information from patients so prescriptions can be filled accurately and efficiently
* Maintain the confidentiality of patients' private medical information according to the HIPPA Act
* Prepare paperwork and compile medications for delivery and mail out orders
* Manage and serve drive-thru patients in an efficient manner (if applicable)
* Ability to log into a variety of software systems and use a cash register
* Stock supplies, clean work area, clean patient area, and prepare pharmacy for next day's operations
* Promote Lewis initiatives and represent Lewis in a positive manner to our patients and communities
* Perform additional duties as assigned by the Chief Pharmacist/Pharmacist

* Ability to lift/move items up to 40 lbs occasionally, up to 10 lbs frequently, and less than 10 lbs constantly
* Visual requirements include vision from less than 20 inches to more than 20 feet with or without correction
* Must have depth perception, color vision (ability to identify and distinguish colors), and field of vision
* Must have ability to communicate verbally with staff and patients

Job Summary

Approval from the Nursing Executive for the market is required in order to waive the bachelor degree requirement (at time of hire). Minimum of two years' clinical experience preferred.

Ability to lift and/or move up to 40-50lbs and push/pull up variable weight limits on flat surfaces or up and down ramps. This position will require the individual to be on their feet for the majority of their shift. May be exposed to certain levels of noise with the equipment in operation.
Depending on location of position, work hours will vary depending on patient/resident needs. This may include overtime, call, backup call, nights, days, weekends, and holidays.
Qualifications
High school diploma or equivalent preferred.
Minimum of six months housekeeping or commercial cleaning experience preferred. Ability to use computer and prior cleaning experience is helpful. Knowledge of materials, equipment, cleaning methods and cleaning chemicals is helpful. Strong customer service, communication and interpersonal skills are required. The Medication Assistant (MA) serves as caregiver who provides resident-centered nursing care and daily living assistance to assigned residents under the supervision of a charge nurse. Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 18 preferred. Prior clinical or nursing assistant experience in long-term care preferred.
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually based on state requirements. Maintains all department specific required certifications. When applicable, may need a valid driver's license and auto insurance.
Iowa: Require Certified Nursing Assistant (CNA) certification with the State Board of Nursing. CNA certification must be maintained with the State Board of Nursing. Successful completion of a state-approved Certified Medication Aide/Assistant course once CNA certification is obtained.
Minnesota: Must have nursing assistant certification, have completed the 75-hour course and be registered with the Minnesota Nursing Assistant Registry. Successful completion of a state-approved Trained Medication Assistant course.
North Dakota: Require Certified Nursing Assistant (CNA) certification with the State Board of Nursing. CNA certification must be maintained with the State Board of Nursing. Successful completion of a state-approved Certified Medication Aide/Assistant course once CNA certification is obtained. If working in an Assisted Living or Basic Care facility, Medication Assistant I certification required. If working in a skilled nursing facility, Medication Assistant II certification is required.
South Dakota: Must successfully complete the state approved Certified Medication Aide (CMA) course. CMA certification must be maintained with the State Board of Nursing.
For all Good Samaritan and Sanford nursing facilities: Must have the state required certified/trained medication aide certification (CMA or TMA). Active certified nursing assistant registration in state of practice, or ability to obtain within 120 days, is required.
