Develops and implements individualized treatment programs for the patients needing services. Reviews plan of care with patient. Uses selected activities and/or tasks to promote and maintain health, and to achieve treatment goals, establish and modify intervention and strategies for patients' treatment program based on ongoing evaluation. Provides direct therapy services to patients using various functional settings and strategies, plans therapy within the patient's individual physical, emotional and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans. Prepares documentation as required by the profession and the department, such as evaluation results, individualized treatment plans, progress notes, and other such reports. Establishes rapport and work cooperatively with interdisciplinary team members (including family/physician) throughout the patient's intervention program. The Administrator is responsible for overall management of Bethel Lutheran Home and carrying out the philosophy and objectives established by the governing board; is directly responsible for general direction and supervision of the Bethel 's financial affairs and transactions, nursing and therapeutic resident services, and maintenance and plant operation; is directly responsible for maintaining compliance with federal, state and local codes, regulations and ordinances as they apply to long term care communities. The Administrator: Develops and implements Bethel Lutheran policies and procedures to comply with Federal, State and Local requirements and to fulfill licensure certification standards. Serves as the primary liaison for the Bethel Lutheran Community with residents, their families, and the general public. The Administrator is referred to for admission decisions of a complex nature, problems with resident's occupancy, waiting list inquiries, etc. Is responsible for Bethel Lutheran operations with regard to staffing, payroll, and benefits administration, in service education, budget review and analysis and operational and capital expenditures. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational. Provides assistance and documentation with wellness and recreational programs and encourages residents to participate. May participate in departmental/organization committees and task forces as needed. May be responsible for completing tasks related to the Restorative Nursing program (including documentation, scheduling and leading restorative programs. Qualifications High school graduate or general equivalency diploma (GED) preferred, but not required. Minimum age of 16. Two years' clinical or nursing assistant experience in long-term care preferred. Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing. Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department specific required certifications. May be required to complete the Restorative Nursing training within 6 months. When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry. Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver's license.
