Northern Light Acadia Hospital Department: Behav Hlth Clinic - Schools Acadia Work Type: Part Time Eligible Hours Per Week: 20 Hours Work Schedule: 7:00 AM TO 2:30 PM The Youth Tele Behavioral Health Program provides students with year-round long term or short term counseling via tele and community resources. The Community Health Worker works alongside a Behavioral Health Clinician and school personnel to help students and families access services and resources. Work closely with Clinician to organize school based behavioral health program and care coordination. The Community Health Worker provides case management; ensuring students and families receive the help they need. Document encounters; collect data; complete monthly reports; maintains comprehensive electronic client files, which include referrals, release of information, consents and other medical documentation.
