Coordinates care planning with other disciplines. May require specialized skills depending on unit assignment. **Duties and Responsibilities** + Provides and coordinates direct care in accordance with the principles of Relationship Based Care to patients based on the nursing process and according to the clinical standards of the hospital. Acts as a primary nurse for patients on the unit. 5 South is a 22-bed medical telemetry unit serving young adults to geriatric patients with a variety of medical diagnoses which may require telemetry monitoring. Admitting diagnoses include, but are not limited to: metabolic disorders, stable cardiovascular, renal, GI bleed, substance abuse withdrawal, diabetes, and respiratory conditions. This unit provides peritoneal dialysis services for all peritoneal dialysis patients within the hospital, and plasmapheresis requiring this treatment. Provides information needed by the registered nurse to coordinate the care and discharge planning for the patient and family. 2. Provides general nursing care and personal care assistance to patients, including bathing, dressing, eating, changing bed linens and bed pans and positioning. 3. Assists in the preparation and transportation of patients for tests, procedures, etc. 4. Monitors patient condition levels; reports changes to the appropriate individuals. 5. Obtains and documents patient vital signs, including temperature, blood pressure, weight, respiration, pulse, pain, and department-specific requirements. 6. Performs selected procedures if demonstrated competency during formal Nurse Extern Program 7. Sets up and removes equipment and supplies; ensures equipment is functioning properly and stored appropriately. 8. Runs special errands as required. Responds to light calls. 9.
