What you will do:
We are looking for a Primary Care Physician interested in providing primary care to underserved communities and join us as Residency Faculty as we enter into the 2nd year of our ACGME accredited Family Medicine Residency Training Program. DVCH is the first and only teaching FQHC in Philadelphia!
As a member of the primary health care team, this position is responsible for rendering professional medical services to the patient population of Delaware Valley Community Health, Inc.. This position provides care to patients with special awareness of vulnerable populations treated in the health centers that may face barriers to care due to residence, health, age, personal characteristics, ability to communicate, chronic illnesses and comorbidities, high ER utilization, developmental status or financial status. The position is also accountable for the complete documentation of patient visits in the patient chart as well as administrative reporting activities as assigned by management.

Graduation from an accredited medical school is required.
Possession of a current license to practice medicine in the state of Pennsylvania is required.
Board certification in specialty is preferred.
Bilingual in Spanish and English preferred.
Experience with treating substance use disorder preferred
Interest in precepting medical students and residents
A minimum of one year experience as a practicing physician in primary health care setting is preferred.
Meets and maintains credentialing and privileging criteria as outlined in the credentialing and privileging policy.

Statement of Purpose:
The Non-Physician Provider is a medical provider responsible to take medical histories, examine patients, maintain patient records, identify health risk factors, prescribe medications, and make referrals and other wise treat health conditions. The practitioner may focus on patient wellness and disease prevention and may provide health education services or counsel patients on how to make healthy lifestyle choices.
Essential Responsibilities:
Perform clinical assessment on new patients, consult to and follow up on patients at the clinic.
Document patients' physical assessments, medical histories, and progress reports under 72 hrs. according to CAN's chart documentation policy.
Render assistance to complete paperwork related to medical management of patients, billing, or credentialing requirements.
Retrieve and evaluate patient's medical results, including pathology and lab results, specialty or primary care consultation and reports on diagnostic tests.
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
Monitor the effectiveness of interventions.
Assess and determine patients' status for symptoms, toxicities, and side effects.
Collaborate and supervise medical staff, as assigned by CAN and according to state law
Provide training to in-house nursing staff.
Collaborate in systemwide educational, clinical effectiveness and quality activities.
Participate in internal and external committees geared to advance the provision of services at CAN
