ABOUT THE JOB
The Client Success Manager is responsible for developing and maintaining a strong and lasting client and broker/consultant partnership with assigned customers. This role is engaged in the strategic and proactive planning within the client's overall line of business and is responsible for identifying and executing opportunities to engage the employee population. This is accomplished via proactive and timely issue resolution, consulting customers on best practices, reviewing service/product offerings, recommending new solutions and providing market insights.
ESSENTIAL DUTIES & RESPONSIBILITIES
* Develop a deep understanding and interest of population health and health care utilization data with the ability to articulate results - leveraging data to tell a story (relate and interpret data).
* Participates in new health center implementations planning and launch alongside the Sales team, Regional Operational Manager and Implementation Team.
* Manage contractual requirements including renewal terms and successful renew clients at targeted escalation. Prioritize low margin clients for improvements in revenue and expense management.
* Identify and target revenue growth opportunities with existing clients including new health centers, products, and services.
* Help define and leverage the "coverage model" for assigned clients.
* Use creative feedback tools to develop an understanding of customer's needs, work with internal partners to improve products and services.
* Coordinate with the marketing team to develop and implement client specific engagement plan including communication and incentive plans to improve utilization and patient experience.
* Responsible for partnering with Regional Operational Managers on ways to drive client engagement, achieve performance guarantees and open communication with clinical staff.
* In collaboration with Operational & Clinical leadership, analyze, summarize, report and manage performance data related to clinic operations. Understand and track risk metrics related to the success of the partnership. Proactively monitor and manage Performance Guarantees to manage payout.
* Salesforce documentation and reporting.
* Execute all necessary contractual requirements in a timely manner; may assist in Requests for Proposals (RFPs) as requested.
* Support the coordination of client eligibility file processing, invoicing, incentive documentation and reporting.
* May serve as the lead point of contact for all customer needs/request.
* Maintain broker relationship and engage in positive relationship to advocate on behalf of mutual client.
QUALIFICATIONS
Bachelor's degree in Business or Healthcare Administration and 5 or more years of directly related healthcare operations or account/client management experience or equivalent combination of education and experience.;

The Home Health Physical Therapist Assistant (PTA, LPTA) performs treatment and provides exercise instruction and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.
Responsible for following all state specific laws governing the provision of physical therapy in home care, to follow the treatment set only as defined by the supervising PT; to perform only those procedures that he/she is qualified and trained to perform and to make all recommendations for changes in the plan of care to the supervising Physical Therapist.
Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition. Coordinates care with the other members of the healthcare team as appropriate.
Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the Physical Therapy Assistant.
License Requirements
Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
Current CPR certification is required.
Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Overview
The Respiratory Therapist II is responsible for medication administration and implementing respiratory care based on expanded knowledge, experience, and the evaluate-and-treat process. The majority of time is in critical care areas with protocol ventilator management, significant independent decision-making and self direction. About the Role
We are currently looking for an experienced Medical Assistant to join our PRN team. Reviews previous medical information and gathers any relevant updated health information from the patient to inform the provider

Prepares and administers approved medications and immunizations via oral, topical, inhaled, intramuscular, subcutaneous or intradermal at the direction of and upon written order from the Provider
Partners with other members of the care team to develop individual patient plan of care including identifying and working to address gaps in care
Front desk duties, including: patient scheduling, medical record requests and abstraction, documentation, coordinate specialist visits, referrals, and other care outside of the clinic, check-in, check-out, etc.
Records accurate and pertinent data in the medical record according to documentation guidelines
Process test results and provide the patient with timely results via e-message, mail or phone as directed in written instructions by the Provider
Participates in outreach to patients to drive engagement among eligible patient groups
Maintains clinical and office supplies and equipment for treatments
About You
Minimum of 1 year of experience working as a Medical Assistant, preferably in Primary Care. May vary based on clinic needs
Graduation from a formal Medical Assistant program or other related program
National or state-specific MA certification or registration is strongly preferred. MA certification or registration is required for employment in States where certification/registration is required.
CPR/BLS certification required at time of start date
Phlebotomy experience is preferred
Pay Range: $20.00-$24.00/hr

Position Summary:
A Clinical Coordinator assists with the daily coordination of personnel and resources within the scope of assignment. Acts as the liaison between the nursing departments and all of the ancillary departments to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency and compliance.
Coordinates staff scheduling, educational/development activities, and performance evaluations.
