Summary:
The purpose of the Clinical RN role is to assess, plan, implement, evaluate, direct, and appropriately delegate to ensure the delivery of safe, quality patient care. This registered professional nurse will function within the full scope of RN licensure, according to the California Nursing Practice Act. Job Description
The RegisteredNurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation, and evaluation. The RN provides direct patient care within the scope of practice and guides patient/family teaching and activities of other nursing personnel while maintaining standards of care in the ICU. The nurse is directly responsible and accountable for the care given to assigned patients, communicates with the physician about changes in the patient's clinical condition, including hemodynamic monitoring, diagnostic studies, and abnormal results. The RN must respond quickly and accurately to changes in conditions and/or responses to treatment while maintaining patient privacy and confidentiality.
Responsibilities
Utilize the nursing process for assessment, planning, implementation, and evaluation of patient care.
Provide direct patient care in the ICU setting.
Communicate effectively with physicians and healthcare team members regarding patient condition.
Teach patients and families about care plans and health management.
Direct and guide nursing personnel in their tasks and responsibilities.
Maintain patient confidentiality and adhere to regulations and standards.
Required Experience / Certifications / Licensure
Current and valid state RN License.
Current BLS (AHA or Red Cross).
Current ACLS (AHA or Red Cross).
Minimum 1 year as a staff RN in an acute care hospital, specifically in critical care.

Provide a complete physical assessment and history of current and previous illness(es)

Writing and initiating plan of care
Regularly re-evaluating patient and family/caregiver needs
Participating in revising the plan of care, as necessary
Initiate the plan of care and make necessary revisions as patient status and needs change
Use health assessment data to determine nursing diagnosis
Develop a care plan that establishes goals, based on nursing diagnosis
Include the patient and the family in the planning process
Record pain/symptom management and changes/outcomes as appropriate
Communicate with the physician regarding the patient's needs and report changes in the patient's condition; obtain/receive physicians' orders, as required
Teach the patient and family/caregiver self-care techniques, as appropriate
Work in concert with the interdisciplinary group
All other duties as assigned
Requirements:
Must have at least one (1) year of acute care experience in an institutional setting
One (1) year of experience in home care preferred
Must have a current licensure in the state
Must understand the philosophy of hospice concept and understand needs of the terminally ill
Must be flexible in work hours
Must demonstrate nursing skills per competency checklist
Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
Ability to travel throughout the Bristol Hospice Service Area

Job Overview
The RegisteredNurse (RN) assumes the responsibility for the application of nursing and clinical processes and delivery of quality patient care at AltaMed clinics under the supervision of the Nursing Supervisor, Nursing Director, or CNE and has accountability for the nursing process and the delivery of efficient patient care. RN supports patient evaluation, and carries out related nursing interventions, supports nursing patient evaluation, efficient system applications, and recording of patient care/behavior. Works collaboratively with the clinic interdisciplinary team members, medical staff, clinical operations, and the Administrative/Operational Leadership teams to ensure the achievement of patient, clinical, or program goals as appropriate. Provides clinical care and demonstrates clinical competency in the area of specialty for patient populations. Application of and knowledge of regulatory requirements in the delegation of tasks and in documentation. Assist in the development and implementation of quality, cost-effective, efficient systems and protocols for standardizing processes of patient care at clinics. Within Health Services, may work as the Triage Nurse in Urgent Care, Primary Care Nurse in the NurseClinic, or the Charge Nurse on the floor at each clinic. Nursing job duties are to comply with the Nurse Practice Act, Title 22, Joint Commission (JC), CMS 42 CFR Part 460 regulatory requirements as appropriate. The plan for the provision of nursing care to patients aligns with the AltaMed Code of Conduct, professional and educational standards, and requirements as needed.
Minimum Requirements
* Accredited school of nursing graduate with a current California State Board of NursingRegisteredNurse License required.
* Minimum of 1 year of experience as a RegisteredNurse preferred
* Experience in an ambulatory care environment, i.e., clinic or Urgent Care setting, preferred.
* Bilingual English/Spanish, preferred. Bilingual in other languages or experience in American Sign Language is highly desirable.
* A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Summary:
The purpose of the Clinical RN role is to assess, plan, implement, evaluate, direct, and appropriately delegate to ensure the delivery of safe, quality patient care. This registered professional nurse will function within the full scope of RN licensure, according to the California Nursing Practice Act. The Clinical RN will provide assessment, development, and implementation of the care plan, evaluating the effectiveness of that care, directing emergency interventions, and collaborating with physicians, team members, and others to optimally manage patient care in the interest of the patient, acting as a patient advocate when necessary.
Job Requirements, Prerequisites and Essential Functions:
EDUCATION
Sufficient education to obtain a valid California RN license
EXPERIENCE
Clinical RN II, either:
a. 6 months to 1-year experience as a Clinical RN I at MarinHealth Medical Center; or
b. Possession of the clinical experience as identified in the appropriate unit -specific job description addendum.
LICENSURE AND CERTIFICATIONS
California RN license Required at hire
Basic Life Support Required at hire
Integrative Agitation Management (IAM) Required within 30 days of hire
PREREQUISITE SKILLS
1. Ability to demonstrate proficiency in all items of the Staff Nurse Skills Criteria at the conclusion of the probationary period
2. Demonstrated leadership and delegation skills
3. Demonstrated oral and written communication skills
4. Demonstrated ability to organize and prioritize work; time management skills
5. Ability to work collaboratively with team members, other teams, physicians, and departments
6. Ability to read, speak, write, understand and follow English verbal and written instructions.
