DuPage County Health Department serves as a Care Coordination and Support Organization and operates as a hub for care coordination for Pathways-enrolled individuals within DuPage County. An individual in the Care Coordinator position will be responsible for providing care coordination and support services to clients enrolled in the Pathways to Success program. The position involves working collaboratively with youth, their families, and community providers to achieve client goals. This full-time position works Monday through Friday, 8:00 am - 4:30 pm, with availability to work at least one evening a week, 10:30 am - 7:00 pm. The hiring range for this position is $44,400 to $52,000 depending on experience. Benefits Galore! This training consists of basic law enforcement skills, immigration laws, firearms training, examination of cargo/bags/merchandise, physical fitness, etc. Candidates assigned to the southern border, Miami, or Puerto Rico duty locations must attend and pass an online Spanish training program, which will be completed at their home port.
