Job Responsibilities and Duties:
Case Managers at Pinnacle Services have the opportunity to work with persons served on CADI, DD, or AC/EW waivers. These persons served may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or are aging.
The Case Manager Is Responsible For:
Assessing the areas of the persons served life where assistance is needed.
Offering and coordinating resources for the persons served being supported.
Monitoring service delivery and working with providers to ensure the individual's needs are met.
Developing service plans.
Attending regular in-person meetings with persons served and their interdisciplinary teams.
Maintaining and Creating accurate Case Notes and Billable Units.
Staying up to date with case management systems, software, and DHS and County resources.
Salary:
Non-LSW wage: $49,000/year salaried for 40hrs/week.
LSW wage: $51,600/year salaried for 40hrs/week.

Our Acute Behavioral Services (ABS) group homes support individuals with complex mental health and behavioral needs. Many of the people we serve have experienced frequent crises, hospital stays, or legal involvement. They may have a history of failed placements and need a highly structured environment to succeed.

Summary:
Be a part of an innovative model that aims to transform the child welfare system, step by step, by bringing substance use treatment into the home, therefore also keeping the child in the home while parents work on their recovery, as opposed to removing a child from the family. A large focus of the day will consist of providing weekly therapy to families in their homes and/or virtually, focusing on the parent child relationship and substance use. This position belongs to a team of 4 therapists and 2 social workers and a clinical supervisor, all of whom will undergo intensive training in the Yale Family Based Recovery Model, weekly consultation, and ongoing clinical support. There is an on-call component to this position on a rotating basis. This work will require ongoing completion of diagnostic assessments, treatment plans and therapy for a small group of families participating in this new approach. This position will maintain compliance with agency, county and DHS requirements.
Therapy Services 60%
Complete diagnostic assessments, treatment plans and ongoing therapy for families involved in child protection
Maintain client caseload that adheres to the model's requirements each month.
Encourage client retention and regular communication, coordinate services with other health care providers to ensure that each client receives culturally aligned and therapeutically appropriate support.
Commit to ongoing professional development opportunities that enhance and strengthen therapeutic practice resulting in client progress and satisfaction with support.
Train in the Yale Family Based Recovery Model and implement it with 100% fidelity
Participate in weekly consultation with the FBR trainer and in team consultations
Assist in the UA collection process of clients

Clinical Therapists (LICSW, LMFT, or LPCC's) are strongly encouraged to apply. What You'll Do:
Provide Hands-On Care: Assist individuals with daily activities like personal care, meal prep, transportation, housekeeping, and laundry, all while fostering independence.

Qualifications
What You Bring to the Table:
At least 3 years of experience working with individuals who have challenging behaviors, such as physical or verbal aggression, property destruction, or self-injury in a caregiving / direct support or relatable role
Must be at least 18 years of age and 21 years or older for some sites
High School Diploma or GED
Passion for Helping Others: Experience with mental health conditions (e.g., bipolar disorder, schizophrenia, depression, anxiety) or individuals with disabilities
Physical stamina to stay on your feet, walk, stand, and lift up to 50 lbs regularly.
Valid driver's license and reliable transportation.
Basic computer skills for tracking progress and documentation.
Good communication is key in this role. You'll rely on your reading, writing, and communication skills to document care, follow support plans, and collaborate with both internal and external team members.

About the Role
As a Unit-Based Services Mental Health Specialist at BrightPath, you will provide person-centered, hands-on support to individuals with disabilities, older adults, and those with mental health support needs in community-based settings. The work is dynamic, meaningful, and best suited for someone who thrives in both independent and team-based settings, is adaptable to changing program needs, and is committed to delivering services with dignity and respect.
This mobile role requires regular travel across the Twin Cities metro area to meet with clients and provide training and shadow shifts for new employees.
Key Responsibilities
Provide Direct Support: Help individuals with physical disabilities, mental illness, and developmental disabilities gain independence by building adaptive skills and supporting daily living needs in community settings.
Develop Emergency Plans: Assist individuals in creating personalized 24-Hour Emergency Back-Up Plans to ensure safety and preparedness.
Deliver Mobile Services: Travel throughout the Twin Cities metro area to meet with clients, provide direct hands-on support, and assist the Designated Coordinator with essential tasks.
Train and Support Staff: Provide shadow shifts and training for new employees to ensure consistent, high-quality service delivery.
Ensure Compliance: Document progress, report incidents, and maintain adherence to Minnesota's 245D regulatory guidelines.
Collaborate Across Teams: Work closely with waivered service providers, mental health teams, social workers, and other professionals to coordinate care.
Crisis Prevention and Response: Play a key role in preventing crises and ensuring appropriate interventions during emergencies.
Participate in On-Call Rotation: Provide 24-Hour Emergency Assistance support on a rotating schedule.
What We're Looking For:
Compassionate, empathetic, and patient, with strong problem-solving skills
Strong organizational and time-management skills
Valid driver's license and access to a reliable, insured vehicle (mileage reimbursement provided)
Ability to pass criminal/registry background checks in accordance with Minnesota DHS guidelines
At minimum, a 2-year degree in Human Services or related field and 2 years of direct experience working with individuals with Intellectual/Developmental Disabilities or related conditions
