What You Will Do
Applies mutually shared and lived experiences navigating the systems of care as a caregiver. Serves as a role model, advocate, and mentor for children and families
Assists with obtaining and maintaining clients' entitlements, including Medicaid, Public Assistance, SSI as well as assisting with problems related to housing or utilities
Interviews clients and significant others, when appropriate, in order to obtain personal histories and to understand the social and emotional factors related to the client's mental health problems
Participates in frequent active engagement with families to engage them in receiving services and assists in empowering them to take the steps necessary to maximize their quality of life
Develops inventory of resources that will meet the clients' needs as identified in the assessment process
Advocates for and assists clients in utilizing adjunctive community resources. Arranges transportation and accompanies clients to appropriate facilities/agencies, as necessary
Provides linkage, coordination with, referral to and follow-up with other service providers. Monitors appointments medical facilities, social service agencies or other providers of concrete services
Collaborates with interdisciplinary team members to discuss care needs and identify solutions to support clients/consumers/members
Attends planning meetings with service providers to coordinate service plans
Initiates written (including clinical and progress notes) and verbal communications, and ensures that all patient record keeping is in compliance with agency, state, city and federal requirements
Participates in agency and community programs and education, as requested
Participates in interdisciplinary team meetings, case conferences, staff meetings, quality assurance meetings, and supervision meetings as required
Qualifications
Licenses and Certifications:
Valid driver's license (i.e., license matching state of residence) required
Credentialed family peer advocate preferred
Education:
Bachelor's Degree in social work, health and human services or a related field preferred
Two or more years of equivalent and relevant experience in a social service agency OR an equivalent combination of the education and experience preferred
Work Experience:
Minimum of one year of experience in a mental health or social service setting with emphasis on family support services and/or family psycho-education required
Lived experience as a caregiver for children with behavioral health needs in accordance with the New York State Office of Mental Health and the New York City Department of Health and Mental Hygiene. required
Bilingual skills may be required as determined by operational needs

RESPONSIBILITIES:

Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.

Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACMTM certification upon entry into this position OR within two years of entry into the position.
CCM or ACMTM certification required OR must be obtained within two years of being placed in the Case Manager II position.
Minimum Qualifications :
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an a ssociate d egree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

Reports To: Coordinator, Functional Family Therapy-Child Welfare Program: Functional Family Therapy-Child Welfare Hours Per Week: 35 FLSA Status: Full Time, Exempt Salary Range: $62,000-$75,000 annually (Bi-lingual candidates may qualify for additional compensation)
Summary Description:
The Functional Family Therapy Child Welfare (FFT-CW) Clinician is responsible for providing FFT services to children and families involved in the child welfare system. This evidence-based practice will be conducted in the family's home setting. The goals of this short-term intervention are to improve family functioning, improve communication, reduce conflict, decrease problem behaviors, reduce hospitalization, reduce out of home placements and delinquency, improve parenting skills, and increase effective problem-solving skills. The Clinician must have the ability to internalize, work within the FFT philosophy, and model as well as being able to adapt and individualize program intervention to utilize family strengths and skills. The Clinician must be eligible and maintain a certification as a Functional Family Therapist Clinician. Training and certification will be provided during the first year of employment. The Clinician will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. The Clinician will utilize this evidence-based practice; always embracing a trauma-informed and person-centered approach.
Key Responsibilities:
Provide direct clinical services to approximately 8-9 families each month which may include, but are not limited to, outreach, assessment and treatment planning, weekly family therapy, crisis counseling and intervention, and community consultation and education.
Coordinate treatment with other community agencies and service providers and participate in meetings as needed.
Participate in program development, training programs and/or meetings as designated by Program Coordinator.
Maintain up-to-date and complete case records, including documentation, and provide written reports and statistics as required.
Complete the required number of client contacts per month.
Follow the established agency policies and procedures and meet all guidelines set forth by the Department of Social Services contract.
Provide 24-hour telephonic crisis intervention services on a rotating basis
Due to the nature of the program, must have the ability to be flexible with working hours.
Maintain appropriate professional standards.
Other designated or related duties.
Key Competencies:
Knowledge of the common challenges associated with mental health conditions.
Knowledge of the common challenges associated with child welfare involved families.
Understanding of the impact of culture on mental health practices.
Familiarity with and ability to effectively use an Electronic Health Record system.
Strong crisis intervention skills.
Excellent problem-solving skills.
Bilingual Spanish preferred.
Key Performance Indicators (KPIs):
Complete all FFT trainings and maintain FFT certification.
Complete documentation accurately and in a timely manner, as per regulatory bodies.
Complete required number of contacts per month.
Requirements:
Must have a Master's Degree in Social Work, Mental Health Counseling or a related field.
Must be eligible for certification as a functional family therapist and complete training within the 12-months post hire.
Demonstrate effective communication skills and the ability to work successfully with a wide range of people.
Must be able to work hours that facilitate meetings with families after school hours.
Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
Ability to document in an electronic health record is required.
Proficiency in Microsoft Office suite and database management systems.
Fluency in English (Reading, Writing, Speaking). Bilingual Spanish a plus.
Must have a valid Driver's License and a driving record that is satisfactory to our insurance carrier.
Must maintain current New York State Defensive Driving certification.
Must be cleared and maintain clearance by applicable regulatory clearances as required.
Must be legally eligible to work in the United States.
Special Considerations:
Hours per week, reporting structure, and location are subject to change per program needs.
This is a mobile position requiring field work.
Must be available to meet with families during after-school hours.
