DUTIES AND RESPONSIBILITIES: Initial and ongoing assessment of client needs in conjunction with clinical providers. Develops and maintains quality, accurate clinical charts for all clients served. Adhere to agency productivity standard. Assist clients in achieving personal independence and managing basic needs as identified by the individual and/or parent or guardian. Facilitation in further development of daily living skills, if identified by the individual and/or parent or guardian. Provide services identified in the Treatment Plan. Assistance with accessing natural support systems in the community. Linkage to formal community services/systems. Participate in organized after school programs as assigned.
