Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
Participate in planning for and the execution of patient discharge experience.

Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
Schedule and facilitate family conferences as needed.
Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
Make appropriate/timely referrals, including documentation to post discharge providers/physician
Ensure accuracy of discharge and payor-related information in the patient record
Participate in utilization review process: data collection, trend review, and resolution actions.
Participate in case management on-call schedule as needed.
Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACMTM certification upon entry into this position OR within two years of entry into the position.
CCM or ACMTM certification required OR must be obtained within two years of being placed in the Case Manager II position.
Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

Evaluates client risk on an ongoing basis and formulates and implements appropriate safety plan for inside and outside sessions.

Rotate on call services.
Video tapes as required for FFT consultation and certification.
Participate in all model and agency training, peer and individual supervision, and staff meetings.
Independently organize and maintain weekly schedule and keep supervisor aware of schedule.
Provide supervised visitation to birth families.
General Agency:
1. Maintains knowledge of and follow Agency policies and procedures as contained in the WCS Personnel Handbook, WCS Procedure Manual, and departmental manuals.

SUMMARYWork as part of the clinical team to provide outpatient individual and group therapy both in general outpatient and intensive outpatient programs. Provide clinical assessment including behavioral health diagnosis and referral for medication management when needed. Perform initial screening, intake and assessment, crisis intervention, provide individual, group, mental health and/or substance abuse therapy, referral for treatment not provided by the program, client education, and treatment planning.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Perform biopsychosocial evaluations/treatments for a panel of clients with mental health and substance use disorders.
Assist the clinical team by providing strengths-based, client-centered, and trauma-informed assessments and treatment to clients with mental illness and co-occurring substance use disorders.
Provide crisis intervention as needed.
Assist with monitoring substance use toxicology screens as needed.
Collaborate with other members of the client's clinical team. Advise and inform clients, staff, or other team members to advance the treatment plan.
Provide therapy services to clients meeting access standards, daily service targets, and quality goals.
Perform other related duties and/or projects as assigned.
Perform record keeping in accordance with Health Information Services and other Center requirements.
Complete daily service documentation.
Provide court reports and other mandated reporting to CT Judicial branch Court Support Service Division
Complete initial assessment forms.
Complete progress notes with appropriate format.
Complete a clear, specific, behaviorally oriented treatment plan with client.
Complete insurance documentation as required.
Complete required reports and evaluations.
Follow required office procedures in a cooperative manner.
Obtain insurance authorization and ensure proper documentation meets quality standards and insurance standards.
Communicate with various local agencies and other departments or teams at The Center.
Work in cooperation with co-workers and supervisory staff.Attend mandatory in-service trainings and other trainings required for renewal of licensure.
Serve on appropriate Center committees as needed.
QUALIFICATIONS
Master's degree in social work, counseling or related field and relevant clinical experience. Behavioral Health Licensure such as: LCSW/LMHC/LPC/LADC required.
Experience providing mental health and substance abuse treatment both group and individual therapy preferred.
Experienced in providing Cognitive Behavioral Treatment, Dialectical Behavioral Treatment, Seeking Safety Treatment, and working with criminal justice population preferred.

Job Summary:
Responsible for providing initial triage and screening of behavioral health services as requested by the Emergency Care Center. Within the emergency setting, provides crisis intervention and assessment to a broad range of psychiatric and substance disordered adults and children, managing patients' needs to appropriate disposition. Qualifications
Educational / Minimum Requirements:

What You'll Do
As a Clinical Therapist in the PRTF Program, you will provide individualized, family, and group therapy to youth in a structured residential setting, while collaborating closely with a multidisciplinary treatment team. You will:
Deliver evidence-based therapy tailored to the complex behavioral, emotional, and psychiatric needs of youth.
Coordinate psychiatric, psychological, and specialized assessments to inform individualized treatment planning.

What We're Looking For

abuse and/or neglect in its various forms, sever family problems and mental health issues.
We are looking for a Therapist for our Functional Family Therapy Foster Care Program to conduct initial assessments including review of referral information, identify and engage key participants, identify protective and risk factors, and develop analysis of hierarchy and relational assessment within the family.

Typical Duties:
Assist in the recruitment of foster and adoptive families.
Conduct appropriate home studies and assist with the placement of children in appropriate adoptive and foster care settings.
Addresses clinical needs of clients through model adherent FFT FC interventions.
Assess strengths/needs of children and families referred for services.
Conduct on going model-specific and foster care adherent risk assessments and interventions for every family including tailored safety planning, crisis management, and interventions that target decreasing risk and increasing protective factors.
Maintain clear and concise model adherent documentation of sessions and collateral contacts that promote monthly supervisory review; documents all contact, collateral notes, sessions, and treatment plans in Care4 database within 2 business days.
Collaborate with foster and adoptive parents as members of the team to protect and nurture children and strengthen families.
In collaboration with the team make decisions regarding permanency plans for children based on assessed strengths and needs.
Identify and coordinate the activities of multiple service providers.
Prepare reports and act as an agent of the agency with the court system.
Plan for termination of agency services.
Education / Experience/Other Requirements:
Master's Degree in Social Work or related field required.
Clinical Licensure (LMSW/LCP/LMFT) preferred or must be license eligible and working towards licensure.
Three years experience in child welfare preferred.
Must maintain a valid driver's license.
Rotating on-call services.

What You'll Do
As SSTRY Clinical Therapist, you will:
Provide individual, family, and group therapy aligned with A-CRA and ACC evidence-based models.
Complete CRA/ACC training and maintain model fidelity.
Collaborate with youth, families, and community partners to assess readiness for change, set achievable goals, and connect to services.

What We're Looking For
Bilingual in English/Spanish required.
Ability to obtain clinical licensure (LADC, LMFT, LMFTA, LMSW, LCSW, LPC, LPCA) in the State of Connecticut
Master's degree in Social Work (MSW), Marriage and Family Therapy (MFT), Counseling, or related field.
1-2 years of clinical experience with youth and families, preferably in behavioral health or substance use treatment.
Knowledge of adolescent substance use, behavioral health, and recovery-oriented interventions.
Ability to engage clients therapeutically and design interventions to promote positive change.
Physical & Work Environment Requirements
Ability to perform light to moderate lifting, standing, walking, bending, and other physical tasks as needed.
Potential exposure to bodily fluids and other environmental hazards in a residential and community-based setting
