Overview
This classification is responsible for professional case management work of considerable difficulty and lead work of routine difficulty; and performs related work as required. This is the lead level class in the DCS Case Manager job series. An employee in this class is responsible for leading and training subordinates in the performance of case management work for children under the State's supervision, in State custody, or at risk of State custody, and their families. This class differs from DCS Case Manager 2* in that an incumbent of the latter performs working level case management work. This class differs from DCS Case Manager 4 in that an incumbent of the latter performs supervisory work over an entire case management team or single/small in an institutional setting.
Responsibilities
Serves as a mentor for less-experienced case management staff by providing guidance and suggestions for difficult/multi-faceted cases. Participates in multi-disciplinary teams including Child Protective Services Investigative Team, Foster Care Review Board, Community Advisory Board, in order to gain a global perspective about case planning decisions.
Determines if abuse or neglect has occurred, who the abuser is, the level of risk or harm to the child, determines the need for a safety plan, makes recommendations regarding permanency and/or termination of parental rights, monitors adoptive and foster families, recruits foster parents, conducts home visits, and coordinates/observes visitation.
Maintains documentation for case files and other required documentation. Prepares for, participates and testifies in court proceedings.
Conducts interviews with alleged child victims, individuals reporting alleged abuse or child neglect, child's family, and collateral contacts; gathers required information such as school records, demographic information, medical exam/psychological information, etc., identifies supports for children and families, administers client drug screens; and conducts required assessments/assessments of home. Conducts face-to-face visits with children, resource families, providers, kinship homes, and other relevant entities.
Works flexible hours including weekends, holidays, and after hours. Schedules appointments, meetings, visitation with children on caseload. Transports children to various appointments safely, assists children in preparing for placement, and sitting with children who are awaiting placement.
Communicates effectively and timely with others, engages with children and families to build trustful relationships, encourages clients to overcome barriers and achieve permanency, explains parental rights and other relevant documents to families, responds timely to families, providers, co-workers and supervision.
Convenes and identifies members for Child and Family Team Meetings to work collaboratively with all Child and Family Team Meeting participants. Develops action steps and goals to be accomplished by the Team.
Works within communities by making referrals to service providers on behalf of clients; collaborates with law enforcement officials and multi-disciplinary teams; trains foster parents, adoptive parents and community.

How you'll contribute
A clinical program therapist who excels in this role:
Coordinates, develops, and delivers curriculum to meet the psychological needs of patients.
Participate within a lead role in the multidisciplinary treatment team planning process, encouraging patient and family involvement.
Perform clinical services aimed at stabilizing the psychiatric crisis and the development of healthy mental health management through a biopsychosocial model of treatment.
Responsible for coordination of discharge plan through discharge planning meetings on assigned units and follows up with patients, family, physician, and hospital staff from admission through discharge.
Initiates patient and family education and discharge planning at admission.
Arranges for discharge and post-hospital care of patients through institutions and agencies within the community.
Coordinates the referral of patients and families to appropriate community, social service, home health service, extended care, and rehabilitation facilities, and organizations.
Creates appropriate discharge plans and documents information in the medical record.
Assists in identifying opportunities for improvement, participates in data collection and analysis, contributes to problem solving and identification of actions to be taken and effectiveness of those actions.
Utilizes evidence-based tools to analyze and identify trends/patterns of performance through variance reporting to improve quality and satisfaction and decrease variation and cost.
Prepares statistical reports on department services and patient needs; participates in the billing procedures for services rendered as required.
Provides education and support to Clinical Staff by attending staff meetings, small groups, and one-on-one education related to resource utilization, discharge planning, and psychosocial aspects of healthcare delivery.
Facilitates group psychotherapy and facilitates psychoeducational focus groups serving as a role model for patients to observe and learn socially effective values, attitudes, and behaviors.
Documents patient care and information in an accurate, legible, manner utilizing correct grammar and spelling; documentation is reflective of patient behavior, thoughts, feelings and responses to milieu and the treatment plan directed interventions.
Interactions with others are nonjudgmental, assertive, and respectful demonstrating a professional, caring attitude and behavior towards others; behaviorally demonstrates understanding of program philosophy, rules, and regulations.
Offer brief individual, group, and family therapy, psychoeducation, crisis intervention, as well as providing information and referrals as needed in collaboration with the case management department.
Reviews charts in preparation of patient treatment planning and provides input on patient progress.
Evaluates and updates treatment interventions in collaboration with charge nurse and/or Program Director.
Demonstrates knowledge of the principles of growth and development over the life span of the patient to include child, adolescent, adult, and geriatric stages.
Serve on weekend assessments, group therapy rotation, and clinical-on-call rotation when needed.

Responsibilities
Provide individual counselling sessions via telehealth
