Duties include, but are not limited to:
* Screening and Assessment: Will work directly with eligible individuals to establish a therapeutic relationship sufficient for completing all required readjustment counseling intake procedures to include an assessment of risk for self-harm and psychosocial functioning. This information will be used as the foundation for identifying the presenting clinical focus areas and for developing the readjustment counseling service plan.
* Counseling Planning/Goal Setting: Develop and periodically update an individualized readjustment counseling service plan that reflects a course of therapeutic and psychosocial interventions, including identification of achievable goals and measurable outcomes.
* Direct Counseling Service Provision: Will implement the individualized counseling plan through an appropriate course of individual, group, couple and/or family readjustment counseling. * Risk Assessment and Crisis Intervention: Will apply risk assessment procedures during the initial assessment and periodically throughout the planned course of readjustment counseling as required and appropriate. For individuals identified from intermediate to high risk, the incumbent will work with the individual to develop a safety plan that identifies the precipitating life stressors and safety procedures identified by the client. Will provide crisis case management services inclusive of coordinating services with the individual's family members.
* Care Coordination: Will establish and maintain a network of community care relationships, internal and external to VA, to coordinate access to readjustment counseling through community referrals to the Vet Center, and to ensure that eligible individuals and/or family members receive the full spectrum of services to support the veteran's readjustment that are outside the scope of readjustment counseling. In support of client care coordination, the incumbent maintains a general knowledge and understanding of the military experience and available VA benefits and services such as VA health care and VBA service-connected benefits.
* Outreach: Will maintain active coordination with Vet Center outreach workers to ensure seamless referral to Vet Center services for those individuals and family members engaged in the community at outreach events that may require follow-up readjustment counseling.
* Team Cohesion and Coordination: Will actively participate in Vet Center staff meetings designed to promote team building and staff development through cojoint planning and problem solving, staff discussion of problematic cases, coaching and mentoring new hires.
* Documentation/Consultation: Will document all clinical interactions with eligible individuals and episodes of care coordination on their behalf as required by RCS policy. Will provide consultation services to other staff inclusive of other community partners.

Perform incidental care to families of the mentally ill and assist those family members in understanding the mental illness.

SUMMARY OF POSITION
As a school-based behavioral health liaison, you will be responsible for providing face to face consultation with classroom teachers to assist them in creating positive, trauma-informed classroom environments, to enhance the learning environment for children at risk of developing an emotional/behavioral problem or substance use disorder, or with serious emotional disturbance (SED). Training and education regarding childhood mental health and trauma-informed behavior responses in the classroom will also be incorporated in the consultation to assist the teachers in promoting a healthy teaching environment. Each SBBHL shall provide psycho-educational groups and other support as needed to youth identified by teachers and/or or other school officials. The SBBHL program will provide Liaison support between the family and the school and will, when invited, assist parents and youth with the Individualized Education Plan (IEP) process and with trauma-informed support plans.
This position is located in the Franklin County School District at South Middle School. Position location may be subject to change based on request and need of school district.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provide face-to-face consultation with classroom teachers to assist in creating a positive, trauma-informed classroom that enhances the learning environment, and to assist the classroom teacher in developing effective, trauma-informed behavior responses for the service population and other students in the class.
Provide at least one face to face training per year to LEA personnel on a variety of mental health and substance abuse prevention topics.
Assist in the development of school wide mental health initiatives and universal prevention strategies.
Provide Liaison services to include communication between the school and the child's family to build open lines of communication and to build home-school partnerships. Liaison services can include assisting in the IEP process when requested, assisting in the alternative school placement process, developing trauma-informed support plans collaboratively with the school and families of the students to meet student individual needs, etc.
Provide information and support for school personnel in navigating the local behavioral health system, including substance abuse and mental health services, and accessing mental health crisis services.
Maintain logs, tracking documents or other appropriate management tools needed to track all aspects of these services. The State will review these documents during the annual monitoring visit.
Ensure that each Liaison provides support opportunities to students at each school to include individual student consultations or interventions to assist in building positive coping and de-escalation skills, as needed; and at least three psychoeducation group activities per year that cover the following topic(s): mental health problems and disorders; the risk of substance use/abuse and substance use/abuse disorders; healthy ways to deal with stress including mindfulness, coping and de-escalation strategies; and other topics as decided and requested by each school.
Address specific mental health or behavioral issues through individual, group, and/or family therapy services. Formulate a diagnosis upon completion of intake assessment. Create and update treatment plans according to policy.
Maintain timely and accurate documentation, including treatment plans, progress notes, and report, to comply with grant and organizational requirements.
Compliance requirement for bi-annual completion of performance evaluation.

: The Children's Outpatient Therapist works onsite at the Cullman campus with our younger patients, their families, and therapeutic team to support clients' growth by utilizing various therapeutic techniques to foster a supportive environment that promotes healthy development and emotional well-being. This is a full-time position working Monday-Friday, 8am-5pm.
What you'll be doing:
Conduct individual, group and/or family therapy to child, adolescent population.
Perform initial assessments, intakes and prepare documentation of such for client electronic medical record, conduct follow up on client's progress.
Prepare with client a treatment plan to include goals with the client.
Prepare documentation with clients of progress and goals to outlined in the Treatment Plan.
Present cases to Psychiatrists regarding client's medication.
Work closely with Case Manager & guardians regarding client's needs.
Provide Crisis Intervention as needed.
Participate in various meetings to include departmental, divisional or other meetings as required by the Director.
As a CANS Super User, assist co-workers that may need remedial CANS Training to obtain certification.
Complete billing documentation/requirements in a timely fashion.
Complete other duties as assigned.
