Duties:
Provides psychosocial assessment to patients and families to identify emotional, social and environmental strengths and problems related to their diagnosis, illness, treatment and/or life situation for those referred by primary care providers.
Formulates, develops and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work and/or counseling treatments and interventions. Interventions may include crisis intervention, brief and long-term individual, marital, and family and/or group therapies.
Provides therapeutic crisis intervention and emergency behavioral health services as required.
Participates in period staff conferences to present case histories; confer with internal external members of the therapy team to develop comprehensive evaluations or plan pertinent therapy programs to provide maximum benefits for each patients.

We are looking to add an Outpatient Counselor to our team in Maquoketa. This role will be responsible for conducting comprehensive assessments and facilitating group and individual sessions for patients seeking assistance with substance use disorders.
As the Outpatient Counselor you will:
Conduct comprehensive substance use evaluations for new patients to determine their individual needs and the level of care required.
Collaborate with community partners to conduct evaluations in person and via telehealth for incarcerated patients.
Complete engagement activities and partner with Supervisor to ensure patients exiting incarceration are assigned a regular Counselor upon release.
Lead group counseling sessions for patients; providing education, support, and guidance in a supportive group setting.
Collaborate with the clinical team on patients' individualized treatment plans based on assessment findings, outlining specific goals and objectives.
Maintain timely, accurate and confidential records of patient sessions, including assessments, treatment plans, progress notes, and group attendance records, in compliance with agency policies and regulations.
Provide urinalysis monitoring on an as needed basis.
