We are seeking a talented [JOB TITLE] to join our team of nearly 500 employees. The Mental Health Specialist, Court Liaison, works with justice involved individuals, who are diagnosed with a mental illness, through their transition into treatment and services in the community. The Court Liaison will have a primary role in Monroe County Mental Health Treatment Court and attend all Court appearances to provide routine progress updates of assigned participants on active caseload. This role requires significant collaboration between the Court systems and local treatment/service systems; attention to deadlines, Court requirements and completion of documentation in a timely manner is essential.
CCSI is partnering with Monroe County Office of Mental Health to hire a Mental Health Specialist, Court Liaison This position is employed by CCSI and working on behalf of Monroe County Office of Mental Health.
In this role, you will
Have a routine presence as liaison in Monroe County Mental Health Treatment Court
Maintain caseload of estimated 35-45 participants
Record progress notes in comprehensive, timely manner and complete all reporting as required by the Monroe County Office of Mental Health and Mental Health Treatment Court
Participate in case review meetings with direct Court staff, as scheduled
Receive assigned cases, review and/or formulate recommendations, submit referrals based on client needs
Recommend adjustments to treatment plans, advocates for equity and system movement to support individual identified needs
Obtain status updates and feedback from direct providers and Court officials to assist with development, and implementation of performance improvement activities for participants
Assess discharge planning needs of individuals being released from local jails, hospitals and other treatment agencies and interact with other personnel as needed for release planning
Communicate in a professional manner, orally, written and/or electronically, as a representative of Monroe County to all Court and community providers and partners
Be culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services.
Have working knowledge of the local behavioral health and social services network to assist with matching individuals with appropriate resources to promote health, wellness, and recovery
Use team approach in working with Socio-Legal Center staff, Courts and community partners.

Requirements:
What You Bring
BSW, BS, BA in Mental Health, Social Work, or Human Services related area with 2-3 years of experience in the behavioral health OR Human Services related field

Ability to satisfactorily complete required background screenings (criminal, SCR, driving, and Monroe County Jail)
Valid Class D New York State Driver License and access to reliable transportation to meet transportation requirements of position

JOB SUMMARY:
Under the general direction of the Director of Behavioral Health Residential Programs, the Behavioral Health Therapist is responsible to provide treatment and therapy for each youth in residence and for their families. The Behavioral Health Therapist coordinates interdisciplinary treatment and completes documentation of all activities in a manner and timeframe consistent with Agency and Corporate Compliance standards. The Behavioral Health Therapist carries out all job responsibilities with fidelity and accountability to the Agency's Mission, Vision, Values, Fundamental Guiding Principles, and Strategic Plan.
ESSENTIAL FUNCTIONS:
Provides crisis prevention and intervention for youth and families.
Coordinates the completion of each youth's Individualized Crisis Support Plan (ICSP) within 72 hours of admission and facilitates its review/revision by treatment team members at every treatment meeting and/or as needed
Provides individual therapy at least weekly to youth in residence
Provides family therapy at least monthly to youth in residence and their family members
Develops written reports on treatment planning and progress for state and local agencies and/or criminal or family courts or makes plans for a designee to do so
Demonstrates, promotes and practices cultural competency toward clients/staff through respect and understanding achieved through training and Agency cultural activities
Completes initial and ongoing assessments of each youth and family's strengths, needs, and progress utilizing information from the youth and family as well as interdisciplinary treatment team members
Complete referrals for youth and/or families to receive specialized clinical treatment services (e.g., chemical dependency, sexual abuse) beyond the scope of practice of the therapist as needed and maintain collateral contact as appropriate, or makes plans for a designee to do so.
Completes progress/service delivery notes and all other required documentation in a manner and timeframe consistent with Agency and Corporate Compliance standards.
Complies with content and timeframe documentation requirements for external reporting specific to program licensing agency (OMH)
Collaborates and communicates with youth/families on an ongoing basis to maximize their leadership and involvement in the residential placement and all services therein

POSITION SPECIFICS:
Education: Master's degree in human service required, Licensure required: LMSW, LMHC, LCAT, LMFT. CASAC or CASAC-T preferred, LCSW a plus!

Work you'll do/Responsibilities
+ Will be joining a very large implementation and build team from Deloitte for this work.
+ Work with the implementation team to plan and complete the build, implement end-to-end Epic module, work command center shifts to investigate during go-live, document, and resolve break-fix tickets.
+ Conduct and document root cause analysis.

Qualifications
Required
+ Epic certification in either Epic Dorothy & Comfort, Hospital Billing or Ambulatory.
+ Minimum of 3 years of Epic build and support experience

A reasonable estimate of the current range is $72,900 to $107,400.

Job Description

This 12-month position is part of the Community-Based Programs Department. The CFT is responsible for providing counseling (individual and family) to youth and their families at identified R-Centers. They also provide crisis response, consultation and information and referral services. Based upon this assessment, a service plan is created that includes the immediate direct services to be provided as well as referrals to other resources in the school and the community. The CFT works closely with the R-Center staff who generate the referrals and assists them to integrate into the R-Center community.

Based upon this assessment, a service plan is created that includes the immediate direct services to be provided as well as referrals to other resources in the school and the community.
